Objective The aim of this study was to investigate the effects of acute myocardial infarction(AMI),stable angina(SA),unstable angina(UA)and coronary atherosclerosis To investigate the serum calprotectin(CAL)levels in patients with coronary atherosclerosis(CA)and to explore the possible clinical value of serum calprotectin in the diagnosis of coronary heart disease. Methods Continuous selection January 2017 to September 2017 period, due to chest pain, chest tightness, palpitations,or suspected coronary heart disease and other reasons, admitted to the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine and cardiovascular medicine and percutaneous coronary intervention(percutaneous coronary intervention 158 patients were divided into coronary heart disease group, coronary atherosclerosis group(sclerosis group)and no stenosis group(control group)according to PCI results. The coronary heart disease group also according to the patient's clinical symptoms and signs were divided into acute myocardial infarction group, stable angina pectoris group, unstable angina pectoris group.Patients with acute myocardial infarction were excluded. The remaining patients were divided into mild, moderate and severe coronary artery stenosis according to the degree of coronary artery stenosis and Gensini score.Acute myocardial infarction patients were taken 0-3h and 3-6h peripheral blood 3ml;the remaining patients were admitted to the hospital after the first blood sample of peripheral blood 3ml,centrifuged and then placed in the upper serum stored in EP tube-40 ? refrigerator.Peripheral blood calprotectin levels were detected by Enzyme-linked immunosorbent assay(ELISA)and the levels of calprotectin were compared by statistical methods. The levels of calprotectin and coronary artery lesion Whether there is a correlation, and its sensitivity and specificity for the diagnosis of coronary heart disease.Result 1.The levels of calprotectin in stable angina, unstable angina, sclerotic and control groups were 25.97 ± 6.72 pg/ml,40.75 ± 17.22 pg/ml,21.00 ± 7.89 pg/ml,18.89 ± 9.21 pg/ml in the unstable angina pectoris group, the highest stable angina pectoris group, the second sclerosis group, the lowest as the control group, and the Bonferroni method was used for comparison between the two groups. The calprotectin level in patients with unstable angina pectoris Group and stable angina pectoris group, sclerosis group, control group differences were statistically significant,P<0.05.2.There was a significant correlation between calprotectin and Gensini score, the correlation coefficient was 0.244,P<0.05.3.Correlation analysis of calprotectin and coronary artery stenosis: Spearman correlation analysis showed that there was a significant correlation between the levels of calprotectin and coronary artery stenosis, the correlation coefficient was 0.478,P<0.01.4.Logistic regression model After adjustment for age,HDL,elevated calprotectin, the risk of coronary heart disease increased(OR=1.178,95% CI:1.087-1.311).5.Prediction of the presence or absence of stenosis of coronary arteries with calprotectin: A sensitivity of 92.9% and a specificity of 80% for coronary stenosis prediction at CAL>22.6 pg/ml.6.Predictive Value of Calprotectin for the Diagnosis of Angina Types: When CAL> 27.05 pg/ml,the sensitivity and predicted specificity of unstable angina were 90.5% and 81.8%,respectively.7.Calprotectin in acute myocardial infarction value: control group and acute myocardial infarction group 0-3h and 3-6h difference was not statistically significant. There was no significant difference in the concentration of calprotectin between 0-3h and 3-6h in acute myocardial infarction. Conclusion Calprotectin levels in different types of coronary heart disease differences, can be used as a prediction of the presence of coronary heart disease, angina type indicators, and its extent and coronary stenosis. However, for the early emergency diagnosis of acute myocardial infarction, the index is not sensitive and specific.
