A Cross-sectional Study On Adherence To Antihypertensive Medication Among Maintenance Hemodialysis Patients

ObjectiveControlling hypertension is essential in chronic kidney diseases.Poor medication adherence is associated with poor disease outcomes,waste of healthcare resources,and contributes to reduced blood pressure control.This study evaluates treatment adherence to antihypertensive therapy in maintenance hemodialysis hypertensive patients by estimating the proportion of adherent hypertensive patients by using a validated tool and investigates what factors predict this behavior.MethodsA questionnaire-based cross-sectional study was conducted on a random sampla of 307 maintenance hemodialysis hypertensive patients selected from hemodialysis units located in Guangdong Provincial Hospital of Chinese Medicine.ResultsThe average mean Morisky medication adherence scale score for the sample was(2.74±1.24),with a range of scores from 0 to 4.Based on the Morisky medication adherence scale scores,37.8%(n=116)of the participants were adherents to medication,and the rest of the participants(n=191)were non-adherents,accounting for 62.2%.The average mean social support rating scale score for the sample was(36.62±5.90),with a range of scores from 23 to 52.86%(n=264)of the participants had good social support,and the rest of the participants(n=43)had general social support,accounting for 14%.The average mean self-efficacy scale score for the sample was(5.95±1.51),with a range of scores from 2.33 to 9.17.The majority(63.2%,n=194)of the participants had general self-efficacy,14.3%(n=44)of the participants had good self-efficacy,and the rest of the participants(n=69)had poor self-efficacy,accounting for 22.5%.Single factor analysis showed that the following variables were associated with medication adherence: age,education level,household income,drug expenditure,daily number of medicines prescribed,daily frequency of taking medication,hypertension duration,years on dialysis,social support and self-efficacy(P<0.05).Spearman analysis showed the following variables were associated with medication adherence: age,education level,household income,drug expenditure,daily number of medicines prescribed,daily frequency of taking medication,hypertension duration,years on dialysis,social support and self-efficacy(P<0.05).Logistic regression showed that the following variables were associated with medication adherence: age,education level,household income,daily frequency of taking medication,hypertension duration,years on dialysis and social support(P<0.05).ConclusionThe medication adherence of maintenance hemodialysispatients with hypertension is suboptimal.Close attention and effective strategies targeting these patients are necessary.Healthcare programs should be designed according to the factors affecting medication adherence.
