Objective:The characteristics of brain structural changes in AD and MCI patients and their correlation with cognitive function were analyzed.Method:We collected 32 AD patients and 10 MCI patients and 40 healthy volunteers from November 2016 to November 2017 in the Department of Neurology and Outpatient Clinic at the First Hospital of Jilin University.Statistical data on the enrolled patients,including general clinical data such as past history,age,gender,education level,etc.,were assessed by neuropsychological assessment staff of the same neurology department,including MMSE,Mo CA,MES,ADL,HIS,CDR,etc.And record the results.All the enrolled members performed magnetic resonance imaging of the 3.0T head,and the engineers used professional software to calculate the volume of each brain area of interest.The enrolled patients were divided into three groups: MCI group(CDR = 0.5),AD group(CDR = 0 ~ 3),and NC group(normal control group).The statistical analysis method was performed using SPSS18.0.Result:1.The age,gender,and education level of the patients were in a normal distribution on the data.There was no significant difference in gender,education level,and age between the NC group,the MCI group,and the AD group(P<0.05).2.The ratio of brain parenchymal volume,bilateral hippocampus,bilateral amygdala volume and total intracranial volume were significantly lower in the AD group than in the NC group(P<0.05).3.The volume of brain parenchyma,the ratio of bilateral hippocampal volume and total intracranial volume were significantly lower in the MCI group than in the NC group(P<0.05).4.The ratio of the volume of the amygdala(right)to the total intracranial volumewas significantly lower in the AD group than in the MCI group(P<0.05).5.The ratio of cerebrospinal fluid volume to parenchymal volume in bilateral frontal lobe,bilateral temporal lobe,and bilateral cingulism was significantly higher in the AD group than in the NC group(P<0.05).6.The ratio of cerebrospinal fluid volume and parenchymal volume in the atrophic part of the temporal lobe(right)in the MCI group was significantly higher than that in the NC group(P<0.05).7.CDR score: There was a negative correlation between the volume of the brain parenchyma,the hippocampal volume on both sides,the volume of cingulate gyrus on both sides and the total volume of the intracranial cavity(%),and the difference was statistically significant(P<0.05);There was a positive correlation between the ratio of cerebrospinal fluid volume and the volume of brain parenchyma(%)on both frontal lobes,bilateral temporal lobes,bilateral parietal lobes,bilateral cingulate gyrus,and bilateral atrophy of insula,and the difference was statistically significant(P <0.05).8.NPI score: with the total volume of the brain,the ratio of the volume of the amygdala(right)and the total volume of the brain(%),was negatively correlated,the difference was statistically significant(P <0.05);with bilateral cingulate gyrus There was a positive correlation between the ratio of cerebrospinal fluid volume and the volume of brain parenchyma(%)in the atrophic part,and the difference was statistically significant(P<0.05).9.MES score: the ratio of total intracranial volume,volume of brain parenchyma to total volume of the brain(%),ratio of volume of hippocampus on both sides to total volume of intracranial volume(%),volume and cranial volume of bilateral amygdala The ratio of total internal volume(%)was positively correlated and the difference was statistically significant(P<0.05).There was a negative correlation between the ratio of cerebrospinal fluid volume and the volume of brain parenchyma(%)between the frontal lobe,both temporal lobes,bilateral parietal lobes,bilateral cingulate gyrus,and bilateral atrophy of insula,and the difference was statisticallysignificant(P<0.05).10.ADL score: with the total intracranial volume,the volume of the brain parenchyma and the total volume of the brain(%),the ratio of the volume of the bilateral amygdala and the total intracranial volume(%),was negatively correlated,the difference has statistics Significance(P<0.05).There was a positive correlation between the ratio of cerebrospinal fluid volume and the volume of brain parenchyma(%)on both frontal lobes,bilateral temporal lobes,bilateral parietal lobes,bilateral cingulate gyrus,and bilateral atrophy of insula,and the difference was statistically significant(P<0.05).11.MMSE score: There was a positive correlation between volume of brain parenchyma,hippocampal volume on both sides,ratio of bilateral amygdala volume to total intracranial volume(%),and the difference was statistically significant(P<0.05);There was a negative correlation between the ratio of cerebrospinal fluid volume and the volume of cerebral parenchyma(%)in leaves,bilateral temporal lobes,both lateral parietal lobes,bilateral cingulate gyrus,and bilateral insular atrophy,and the difference was statistically significant(P<0.05).12.The MOCA score was positively correlated with the volume of brain parenchyma,the hippocampal volume on both sides,the ratio of the volume of amygdala on both sides and the total intracranial volume(%),and the difference was statistically significant(P<0.05);with both frontal lobes There was a negative correlation between the ratio of cerebrospinal fluid volume and the volume of brain parenchyma(%)on both sides of temporal lobe,bilateral parietal lobe,bilateral cingulate gyrus,and bilateral atrophy of insula,and the difference was statistically significant(P<0.05).Conclusion:1.Brain atrophy can occur in patients with MCI,first appearing in the right temporal lobe and hippocampus on both sides.As the disease progresses,the patient's brain atrophy gradually aggravates,and the frontal lobe,amygdala,cingulate gyrus and other parts are gradually involved.2.As the degree of atrophy in each brain area gradually increases,the degree ofdementia gradually increases.3.Atrophy in different brain areas of patients has a certain correlation with different cognitive impairments.
