Objective:Of this study is to through the comparison between OSAHS patients and normal control group serum Vaspin levels and metabolism related data,to study the serum Vaspin and the relationship between OSAHS patients with insulin resistance.Furthermore,the possible roles and mechanisms of Vaspin in OSAHS with metabolic syndrome(MS)were discussed.Methods:A simple random sampling method selected 30 healthy people from the physical examination center of the Second Affiliated Hospital of Kunming Medical University from January 2017 to March 2018 as a control group.In the experimental group,69 patients with OSAHS were divided into two groups:mild group(34 persons)and severe group(35 persons).All subjects completed the Polysomnography(PSG)and recorded the AHI and the lowest SaO2.All subjects were tested for fasting serum Vaspin,fasting blood glucose(FPG),fasting insulin(FINS),triglycerid(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholestero(HDL-C),uric acid(UA),et al.All patients measure height,body weight,neck circumference,waist circumference,hip circumference,and calculate waist-to-hip ratiobody,body mass index(BMI),homeostasis model assessment of insulin resistanc(HOMA-IR).By analyzing the relationship between Vaspin and the above mentioned metabolic factors in the experimental group and the control group,the correlation indexes were screened.Furthermore,to evaluate the relationship between Vaspin level and insulin resistance,and explore the possible mechanism of Vaspin in OSAHS and MS.Results:There was no statistical difference in general clinical data such as height,sex and age between the control group and the experimental group(P>0.05).In the control group and in the two experimental groups,body weight,waist circumference,waist-to-hip ratio,BMI and AHI was increased,and the lowest SaO2 decreased successively(P<0.05).The neck circumference of severe group was higher than that of control group(P>0.05),but there was no significant difference in severe group to mild group,and mild group to control group(P>0.05).Compared with LDL-C in the three groups,there was no statistical difference(P>0.05).The FINS.HOMA-IR increased in the control group and the two experimental groups,and the serum Vaspin decreased(P<0.05).The level of TC.FPG in severe group was higher than that in mild group and control group(P<0.05),but there was no statistical difference between mild group and control group(P>0.05).HDL-C was lower than that in mild group and control group(P<0.05),but there was no difference between mild group and control group(P>0.05).The UA in severe group was higher than that in control group(P<0.05),but there was no significant difference between severe group to mild tgroup,and mild group to control group(P>0.05).Pearson correlation analysis showed that there was no significant correlation between serum Vaspin and sex,height and TC,but a positive correlation was found between serum Vaspin and LSa02,HDL-C.A negative correlation was found with age,body weight,BMI,neck circumference,waist circumference,hip circumference,waist-to-hip ratio,AHI,TG,LDL-C,FPG,FINS,UA,HOMA-IR.The results of multiple regression analysis showed that:AHI=56.201+28.776waist-to-hip rati-0.707LSa02-0.370Vaspin.Conclusions:1.The level of serum Vaspin in patients with OSAHS was lower than the healthy control group with statistical significant,and the more severe of OSAHS,the lower the level of serum Vaspin.2.OSAHS in patients with metabolic syndrome incidence rate was higher than the healthy control group,the more severe the OSAHS,the higher incidence of MS.3.Serum Vaspin is negatively correlated with insulin resistance and plays an important role in OSAHS and MS.4.Serum Vaspin can be used as one of the indexes to detect the severity of OSAHS.
