Objective:Auditory steady-state responses(ASSR)means the phonic stimulate signal of multiple frequencies of continuous and homeostatic stimulate the organ of hearing,thus record a set of steady electrical responses of the brain on the scalp of the subjects.This examination has a certain value of assessment in the examination of audiology for severe and extremely severe sensorineural deafness before cochlear implantation.Neural response telemetry(NRT)is a technique for recording the electrically evoked auditory nerve compound action potential(ECAP)in the cochlea.Now it has been widely applied in intraoperative detection,postoperative start-up and debugging.At present,accompanied by the intensive study of cochlear implantation,the indications of the operation are also gradually expanding.Some patients with inner ear malformations can also improve their hearing through surgery.This artical analyses the examination results of 111 children with cochlear implants,discussing the relevance of ASSR,the anatomy of the inner ear and T-level in one months,six months and one year after operation.Methods:Taking the children with pre-lingual deafness who accepted cochlear implantation in first Affiliated Hospital of Kunming Medical University from january 2015 to december 2016 as the research object..The total number of children was 111 cases,the age is from 0.9 to 9.1 years old.According to the anatomical structure of the inner ear,it is divided into two groups,the anatomy of the inner ear is normal in group A,and the abnormal in group B.According to whether is there residual hearing of AS SR before operation,the group A was divided into the residual hearing group and the non residual hearing group,among them,0.5KHZ stands for low frequency,1KHZ and 2KHZ for midfrequency and 4KHZ for high frequency.the two frequencies of the midfrequency can be induced or one of the two frequencies can be induced represent have residual hearing,two frequencies are not elicited represent of no residual hearing.On account of there was little residual hearing in the group B,select the same number of children with the group B from the children with no residual hearing in group A as the control group of group B--group C.Determination of T-level in each group on the first,the eleventh and the twenty-second electrode in one months,six months and one year after operation.the 1st electrode,the 11st electrode and the 22st electrode separate representation the high frequency section,intermediate frequency section and the low frequency section.Compare:?The difference in T-level between residual hearing groups and no residual hearing groups in group A.? The difference in T-level between group B and group C.Results:1.In the group A,compare the T-level between the residual hearing groups and the no residual hearing groups in one months and six months after operation,a part of P<0.05 and another part of P>0.05.So there is no clear statistical significance.See the table 1 and the table 2.2.In the group A,the T-level in the residual hearing group is less than the T-level in the no residual hearing group in one year after operation.The difference was statistically significant(P<0.05).See the table 3.3.The T-level in the group C is lower than the T-level in the group B in one months,six months and one year after operation.The difference was statistically significant(P<0.05).See the table 4,table 5 and the table 6.Conclusion:Through the research and analysis of this subject,we can get the following conclusions:?There is a correlation between the ASSR before operation?the anatomy of the inner ear and the T-level after operation.For the children who prepared for cochlear implantation,the ASSR and imaging examination can help doctors and parents predict the effect of hearing reasonably,and provide a reasonable expectation for them,so then,it is helpful for children to receive more timely and effective speech rehabilitation training after operation.? ASSR can evaluate the preoperatively residual hearing,and it also can be used as a reference for clinicians to select the ear for surgery.
