The Summary Of Qi Yuru's Academic Experience And The Research Of Dialectical Treatment Rule Of Pogejiuxin Decoction

ObjectivesQi Yuru is a famous doctor of Shaanxi Province,the apprentice of Shaanxi famous TCM doctor Li Ke.After inheriting the experience of his master,he gradually established his academic thoughts and clinical experience in treating emergency severe cases and miscellaneous diseases in the clinical practice.The dialectical treatment rule of Pogejiuxin Decoction used by Qi Yuru is an important part of his thoughts and experience.This article attempts to explore the dialectical treatment rule of Pogejiuxin Decoction through data mining.It is conducive to inherit and develop Qi Yuru's thoughts and experience.MethodsThe 201 clinical cases of Dept.5 of internal medicine of Fangcun branch of TCM hospital of Guangdong Province from 1^st April 2012 to 31^st June 2015 were retrospectively analyzed.The general information,symptoms,tongue,pulse and herbs were collected into the database to explore the dialectical treatment rule of Pogejiuxin Decoction through frequency and cluster statistics.ResultsBased on data mining,the article retrospectively explored the dialectical treatment rule of Pogejiuxin Decoction used by Qi Yuru.The results shows that:?1?This decoction is mainly used to treat old patients with chronic diseases caused by cold-evil.Most of the courses of the diseases are half a year.The average age of all the patients is 68.60±15.50 years old.Most of the patients are 70-85 years old.The male to female ratio of cases is neatly 1:1.This decoction is widely used to treat 33 diseases of TCM like asthma,heart failure,Lung-heat disease and lung inflation.And it is also used to treat 66 diseases in western medicine like respiratory failure,heart failure,COPD and cerebral infarction.?2?The main symptoms,tongue and pulse of Pogejiuxin Decoction are:spiritlessness,insomnia,poor appetite,asthma,phlegm,cough,pale dingy tongue,white greasy fur,deep weak wiry slippery pulse on both hands.?3?It divided all the prescriptions into 7 groups by cluster analysis.In Group3,it used 120g cooked aconite,and the dose of dried ginger was larger than or equal to prepared licorice?60-90g dried ginger and 60g prepared licorice?.The percentage of floating pulse on left hand has increased,while the percentage of deep slippery pulse on left hand and slippery pulse on right hand has decreased.In Group 1,it used 120g cooked aconite,and the dose of dried ginger was smaller than prepared licorice?60g dried ginger and 90-120g prepared licorice?.The percentage of deep wiry slippery pulse on left hand and thread wiry pulse on right hand has increased.In Group 6,it used 200g cooked aconite,and the dose of dried ginger was smaller than or equal to prepared licorice?60-120g dried ginger and 90-120g prepared licorice?.The percentage of asthma and rapid pulse on both hands has increased,while the percentage of poor appetite,insomnia,deep weak pulse on left hand and deep wiry slippery pulse on right hand has decreased.In Group 2,it used 30-45g raw aconite,and the dose of dried ginger was smaller than prepared licorice?60g dried ginger and 90-120g prepared licorice?.The percentage of thirsty,slimy fur,weak wiry slippery pulse on left hand and wiry slippery pulse on right hand has increased,while the percentage of phlegm,cough and deep pulse on both hands has decreased.In Group 7,it used 30-60g cooked aconite,and the dose of dried ginger was smaller than or equal to prepared licorice?45-60g dried ginger and 45-60g prepared licorice?.The percentage of spiritlessness and slippery pulse on right hand has increased,while the percentage of asthma,greasy fur and weak pulse on right hand has decreased.In Group 4,it used30-60g raw aconite,and the dose of dried ginger was larger than prepared licorice?90-120g dried ginger and 60-90g prepared licorice?.The percentage of spiritlessness,poor appetite,deep pulse on left hands and deep weak pulse on right hand has increased,while the percentage of wiry slippery pulse on right hand has decreased.Group 5 was not discussed in this passage because the sample size was not enough.?4?In modified drugs,amomum,poria,aquilaria,pinellia,fried barley sprout,flying squirrels droppings,epimedium,lyceum,psoralea,cuscuta and yam are always used together with Pogejiuxin Decoction.The modified drugs in Group 3 are consistent with the overall results.In Group1,the usage rate of aquilaria,alisma and amomum has increased.In Group 6,the usage rate of poria and scallion white has increased while the usage rate of pinellia has decreased.In Group 2,the usage rate of pinellia,flying squirrels droppings,fried crataegus,fried barley sprout,fried rice sprout,fried medicated leaven,fresh ginger and poria has increased.In Group 7,the usage rate of poria,aquilaria,flying squirrels droppings and amomum has decreased.In Group 4,the usage rate of asarum,epimedium,lyceum,psoralea,cuscuta,flying squirrels droppings,cinnamon twig,peony and ovate atractylodes has increased.ConclusionBased on the framework of“Qi-Yin Yang-Three Talents-Four Images-Five Elements-Six Climates”structured by Qi Yuru and his academic thoughts and clinical experience,the article retrospectively explored the dialectical treatment rule of Pogejiuxin Decoction used by him.The results shows that:?1?This decoction is mainly used to treat old patients with chronic diseases caused by cold-evil,especially for respiratory and circulation failure.?2?The pathogenesis is deficiency-cold of kidney,spleen and heart combined with phlegm and blood stasis.?3?The result of medication rule is that:Raw aconite is mainly used for deficiency of yang while raw aconite is mainly used to reduce phlegm and blood stasis or strengthen yang.Small,middle and large dose of cooked aconite are respectively used for mild,severe deficiency of yang and to preventing Promordial Qi from breaking away.Larger dose of prepared licorice is used for deficiency of yang or to reduce phlegm stasis,while larger dose of dried ginger is used to treat exterior syndromes and transport spleen.?4?The modified drugs possesse three effects including tonifying and transporting spleen,tonifying and absorbing Qi to kidney and eliminating evil.
