ObjectiveThrough randomized controlled trial,we observed the difference of clinical efficacy between different acupuncture methods in the treatment of acute scapulohumeral periarthritis,so as to provide an optimal treatment for acupuncture treatment of acute periarthritis of shoulder.Methods108 cases of acute periarthritis of shoulder were selected according to the inclusion criteria of this study. All patients were randomly divided into three groups : conventional acupuncture group(conventional group),electroacupuncture group and warm acupuncture group, each group was 36 cases. The conventional group was treated with ordinary acupuncture, and the electroacupuncture group was connected with the density wave on the needle after acupuncture, with a KWD-808 I electroacupuncture treatment instrument with a frequency of 2 to 100 Hz.The warm acupuncture group was connected to the needle handle with the size of 1.8cm x 2.7cm column as a heat source for warm moxibustion after acupuncture gets gas.Acupoints that we seclected were all the affected side, included Jian yu,Jian zhen,Jian liao,Jian qian,Ashi,He gu,Qu chi,Wai guan, and the treatment courses were also consistent. The difference of VAS scores and Constant-Murley scores of three groups were compared before and after treatment, and the difference of clinical effect between the three groups was compared.Results1.VAS scoring:There was no significant difference in the VAS scores between the three groups before treatment(P>0.05),but the difference of VAS scores were statistically significant after treatment(P < 0.01).The difference of VAS scores in three groups were statistically significant before and after treatment(P<0.01),and the difference of VAS scores betweenthe two groups were statistically significant before and after treatment(P<0.01).Compared with the conventional group, the difference of VAS score and VAS score of thermoacupuncture group electroacupuncture group were statistically significant(P<0.01);Compared with the electroacupuncture group, the difference of VAS scores in the warm acupuncture group were not statistically significant(P>0.05).2.Constant-Murley scoring:There was no significant difference in the Constant-Murley scores between the three groups in the acute stage of scapulohumeral periarthritis before treatment(P>0.05),but there was a significant difference in Constant-Murley scores after treatment(P<0.01).The scores that before and after treatment within the group were statistically significant difference(P < 0.01),and the difference of Constant-Murley scores between groups was statistically significant(P<0.01).Compared with the conventional group, the difference of Constant-Murley scores in electroacupuncture group and warm acupuncture group were significant statistical significance(P<0.01),compared with the electroacupuncture group, the difference of Constant-Murley score in warm acupuncture group were significant statistical significance(P<0.01).3.Clinical efficacy: The total effective rate of the warm acupuncture group, the electroacupuncture group and the control group were94.29%,91.43%,and85.71%respectively,and the difference of clinical curative effect between the three groups were statistically significant(P<0.05).Compared with the conventional group, the clinical curative effect of electroacupuncture group had statistical difference(P<0.05),and the clinical efficacy of warm needling group had significant statistical difference(P<0.01).Compared with the electroacupuncture group, there was no significant difference between the clinical efficacy of warm acupuncture group(P>0.05).ConclusionThree different methods of acupuncture and moxibustion treatment for acute periarthritis of shoulder could all obtain good clinical effect, all of them were a method of high safety and no adverse reaction. In the improvement of patients with shoulder joint pain and functional activities, the clinical curative effect of electroacupuncture and warm acupuncture was better than that of conventional acupuncture; In terms of relieving shoulder pain, therewas no obvious difference between warming acupuncture and electroacupuncture. However,in improving shoulder function, the clinical effect of warming acupuncture was better than electroacupuncture. Therefore, the effect of warm acupuncture on patients with acute scapulohumeral periarthritis is better, which can effectively relieve pain, improve the activity of the regulatory agency and improve ADL,which is better than electroacupuncture and ordinary acupuncture.
