Objective: To inventigate the principle of self-developed multifunctional fracture reduction oriented forceps treating long bone shaft fractures in the minimally invasive surgery t and to evaluate the early clinical effect.Methods: From October 2013 to July 2014,24 cases with distal fibula fracture were treated with conventional open reduction and internal fixation therapy(ORIF group),including 13 males and 11 females with an average age of 44.9 years old(20 to 74 years).From August 2014 to March 2015,22 cases with distal fibula fracture were treated by minimally invasive surgery with the self-developed multifunctional fracture reduction oriented forceps(Percutaneous minimally invasive group).There were 12 males and 10 females with an average age of 45.5 years old(20 to 74 years).In the percutaneous minimally invasive group,the reduction of the three-dimensional displacement of the fracture was reduced to the reference template of the two-dimensional plane.Small incisions were made in both sides of the fracture,forming a subcutaneous tunnel.A Locking compression plate was inserted outside the periosteum,and at each end of the plate two self-developed multifunctional fracture reduction oriented forceps were fixed.The forceps were fixed by traction and rotating reduction.Through the rapid pressure cylinder of the forceps,the segments were pulled to the plate.When the circular cross-section of each segment was tangent to the plate,the fracture has been fully restored and temporarily fixed.The incision length,operating time,blood loss,fracture healing time and American Orthopaedic Foot and Ankle Society(AOFAS)score between the percutaneous minimally invasive group and the ORIF group were evaluated and compared.Results: All cases were followed at least 1year.The percutaneous minimally invasive group and the ORIF group had statistically significant differences in incision length(7.09 ± 0.76 cm VS(8.79 ± 0.66 cm,P<0.05),operating time(32.50 + ±4.89 min VS 39.17 ±4.28 min,P<0.05),blood loss(8.01 ±2.71 ml VS30.95 ±11.35 ml,P<0.05),and the fracture healing time(16.41±2.28 weeks VS 19.50±2.86 weeks,P <0.05).difference.About limb function American Orthopaedic Foot and Ankle Society(AOFAS)score,18 cases in Percutaneous minimally invasive group were excellent,3 cases good,1 cases fair,0 cases poor;and 20 cases of the ORIF group were excellent,3 cases good,1 eases fair,0 cases poor.There was no significant difference.Conclusion: The use of self-developed multifunctional fracture reduction oriented pliers in the treatment of long bone fractures has the advantages of simple operation techniques,saving operating time,reducing soft-tissue damage and promoting fracture healing.
