Objective:1.To investigate the protective effect of low-molecular-weight-heparin gel(LMWH gel)on ischemia-reperfusion injury(Ischemia/Reperfusion,I/R)in rat skin flap.2.To investigate the possibility and mechanisms of microneedle combined with low molecular weight heparin gel in the prevention and treatment of ischemia/reperfusion injury in skin flap.Methods:1.Experimental animal grouping and model preparation:60 male rats were randomly divided into 5 groups,group A,group B,group C,group D and group E,with 12 rats in each group.2.Experimental methods:Right low abdominal island flap which fed by the superficial epigastric artery were created in rats of all 5 groups.group A(sham-operated group):no perfusion intervention;the remaining four groups were given 4 hours of ischemia.After that,group B:no treatment,group C(micro needle group):microneedles treatment after operation,group D(LMWH gel group)treated with low molecular heparin gel smear,E group combined with low molecular weight heparin gel and microneedle intervention at the same time.3.Testing index:Histopathology observation.Detect the SOD,MDA,NF-?B in every group by relevant kits.4.Calculating the surviving rate of flaps 7 days after operation.Results:1.Histopathological observation showed that the degree of edema and infiltration of inflammatory cells in the skin flaps of group D was significantly lower than that of group B.Compared with the group D,the degree of flap edema and infiltration of inflammatory cells was reduced after the intervention of microneedle combined LMWH-gel.2.The expressions of MDA in the flap:group C>group B>group D>group E>group A(P<0.05);the expressions of MDA in the flap:group A>group E>group D>group B>group C(P<0.05).3.Western blotting analysis and the result of immunohistochemical experiments both showed that the level of NF-?B expression:group C>group B>group D>group E>group A(P<0.05).3.5 days after operations,the survival rate of the skin flap was detected:group A>group E>group D>group B>group C(P<0.05).Conclusions:LMWH gel can effectively protect the skin flap,reduce the inflammatory response and improve the survival rate of the flap.Microneedle combined with LMWH gel can improve the protection effect of LMWH-gel in flap I/R injury.
