Clinical Significance Of CEA,CA724,and CA125 Combined Detection On Gastric Cancer

Objectives:In this study,we made analysis on CEA,CA724,and CA125 detection of serum in 143 cases of patients with gastric cancer and 149 cases of patients with benign gastric disease,and discussed the relation between the expression level of serum tumor markers and the screening and diagnosis of early gastric cancer.Furthermore,we discussed the clinical significance of the three abovementioned serum tumor markers on gastric cancer patients of varied differentiations,stages and types.Methods : 143 cases of gastric cancer patients were selected from the Second Hospital of 2014-2016 years,including 81 cases of male and 62 of female,with an average age of 66.3 years;According to the standard of different pathological differentiation: Among which there are 56 cases of high and medium differentiation,and87 cases of low differentiation;According to the AJCC clinical staging criteria for gastric cancer of the United States: There are 75 cases in phase I,or II;36 cases in phase III,and 32 cases in phase IV.According to histological classification: There were99 cases of papillary adenocarcinoma,24 cases of signet ring cell carcinoma,2 cases of tubular adenocarcinoma,4 cases of mucous adenocarcinoma,and 14 cases of special type of gastric cancer.At the same time,there were 149 patients with benign digestive tract diseases,including 71 males and 78 females,with an average age of 61.9 years.There were 48 cases of chronic atrophic gastritis,51 cases of gastric ulcer and 50 cases of gastric polyps.All the patients have been diagnosed by endoscopic biopsy,anddiagnosed by pathological examination.Tumors in other parts of the patients with benign digestive tract lesions have been excluded.51 healthy examiners in our hospital were selected as the control arm.A retrospective analysis was carried out,applying statistical methods including chi-square test,t test and ROC curve.Results:1.The levels of the three tumor markers in the benign gastropathy group and the healthy control group were compared: The serum levels of CEA,CA72-4 and CA12-5in patients with chronic atrophic gastritis,gastric ulcers and gastric polyps were higher than those in the healthy control group(P < 0.05).2.The positive rates of the three tumor markers in the benign gastropathy group and the healthy control group were compared: The positive rate of serum CEA,CA724 and CA125 in the patients with gastric ulcer,gastric polyp and chronic atrophic gastritis was higher than those of the healthy control group(P < 0.05).3.The levels of three tumor markers in the gastric cancer group and the benign gastropathy group were compared: The level of CEA,CA724 and CA125 in the gastric cancer group was significantly higher than those in the benign gastric disease group(P <0.05).4.The positive rates of three tumor markers in the gastric cancer group and the benign gastropathy group were compared: The positive rate of serum CEA,CA724 and CA125 in the patients with gastric cancer was significantly higher than those in the benign gastric disease group,which showed statistical significance(P < 0.05).5.The levels of the three tumor markers in patients with different differentiation and staging of gastric cancer were compared: The level of serum CA724 in the low differentiation group was significantly higher than those in the middle and high differentiation group(P < 0.05),and there was no difference in the level of serum CEA and CA125 in the two groups(P > 0.05).In the comparison among gastric cancer patients of different clinical stages,the serum levels of the three tumor markers in patients with phase IV gastric cancer were higher than those in patients with phase I or II gastric cancer(P < 0.05).However,there was no significant difference in serumtumor markers between phase III and phase IV patients(P > 0.05).6.The positive rates of the three tumor markers in patients with different differentiation and staging of gastric cancer were compared: There was no difference between the positive rate of the three tumor markers in the high differentiation group and the low differentiation group(P > 0.05).The positive rate of the three tumor markers in patients with phase IV gastric cancer was significantly higher than those in patients with phase I or II gastric cancer(P < 0.05).However,there was no difference in positive rate of the markers between phase III and phase IV patients.7.The levels of the three tumor markers in patients with different classification of gastric cancer were compared: There was no difference in serum levels of CEA,CA724 and CA125 in patients with gastric papillary adenocarcinoma,mucous adenocarcinoma,tubular adenocarcinoma,signet ring cell carcinoma and special type of gastric cancer(P > 0.05).8.The sensitivity and specificity of single detection and joint detection of the three tumor markers in patients with gastric cancer were compared: The single detection sensitivity of the three tumor markers,namely CEA,CA724 and CA125,in the gastric cancer group were 40.6%,44.1% and 37.1%,respectively.While the sensitivity of combined detection was 49.7%.Thus it can be well concluded that the sensitivity of combined detection was significantly higher than that of single detection.Conclusions:The increase of serum levels of CEA,CA724 and CA125 is closely related to the occurrence and evolution of gastric cancer,and to a certain extent,it reflects the progress of gastric cancer.The combined detection of these three serum tumor markers is more helpful to the early diagnosis,treatment management and prognosis of gastric cancer.
