Objective:To discuss the initial stability difference between tilted and non-tilted implant.Materials & methods: This study include 10 full mouth implant restoration cases treating from October 2015 to June 2017 in HBO Implant center located in Taiwan.10 cases were selected for All on 4 or All on 6 implant surgery in this study.All the cases were operated in both upper and lower arch,include 8 males and 2 females.The 10 patients who are participating within this research are between age 40-68.The 10 patients are all choose All on 4 or All on 6 implant restoration surgery,which is also known as immediate implant and immediate loading surgery.The disadvantage of traditional implant method that takes long time for bone growth,such as sinus lift GBR surgery,and limits implant place to avoiding damage of mandibular nerve.As experimental group,we have tilted implant at #15.#25.#35.#45 to avoid sinus augmentation and mandibular nerve injuries.For control group,we have the same patient with non-tilted implant.Before surgery,we will do routine oral examination,take Cone Beam CT,and extra oral examination like facial diagnosis.We will fully discuss with patients regarding the difference between traditional implant treatment and All on 4 immediate loadingimplant,after understanding the pors and cons then the patient get to decide the choice of the treatment of their choice.After estimating the maxilla residual alveolar bone high and mandible nerve position to choose the implant type rather tilted or non-tilted implant.Using the Cone Beam CT to plan the angle and the position of all the implant.Cooperating with prothodontist,analysis the final result of the correlation between the face and the teeth when patient smile and laugh to decide the optimal teeth position in the mouth.Two weeks after surgery,the first time Periotest will be done to check the stability of tilted and non-tilted implant.The second Periotest examination will take place three months after surgery.The third Periotest examination will take place six months after surgery.Evaluate the outcome of implant surgery and dental restoration by using Periotest examination,the Cone Beam CT scan after six months,and patient satisfaction survey.Results:This study will conclude the result of Periotest be done to check the stability of tilted and non-tilted implant.80 implantation of all.After three to six month of osseointegration,all implants in experimental group and in control groups are having no infection,losing,dislodging,and bone resorption after 3 to 6 months by having Cone Beam CT examination.Two weeks after surgery,the first time Periotest be done.The PTVs of tilted implant is-1.5 to +7.9.The PTVs of non-tilted implant is-6.1 to +13.8.At the first time,the stability of tilted and non-tilted implants are decide on the insert torque.Therefore the stability of tilted and non-tilted implants are not such obvious difference.Three months after surgery,the second Periotest be done.The PTVs of 36 tilted implant is minor than non-tilted implant.(The stability of 90% tilted implant is better than non-tilted implant.)One to three month after surgery is the mean time of the osseointegration.At that time,the stability of tilted implant is apparently better than non-tilted implant.Six months after surgery,the third Periotest be done.The PTVs of 39 tilted implant is minor than non-tilted implant.(The stability of 97.5% tilted implant is better than non-tilted implant.)There are 4 non-tilted implants PTVs are between 0 to 4.Other 76 implants PVTs are between-8 to 0.95% implantations have good stability and good osseointegration.The osseointegration and stability of the implants are good by using Periotest examination.Most of the implantation are well,but some of the implantation have inflammation around the cervical of the implant may probably cause by terrible oral hygiene.90% satisfied with the treatment by the patient satisfaction survey.Conclusion: This study utilized tilted implant and non-tilted implant on All on 6 and All on 4 immediate loading restoration cases with six months follow-up.We control the insert torque of the implantations to reduce the cause of stability.Following up these tilted implant and non-tilted implant 3 to 6 months after surgery,we find that most of the tilted implants have better primary stability than non-tilted implant.Future clinical follow-up and references are still required to support this theory.
