Purpose:To compare the safety and efficacy of 27+ G,25+ G in the clinical application of transconjunctival sutureless vitrectomy(TSV).Methods:A retrospective analysis of 76 cases(76 eyes)from Xiamen University Ophthalmic Center affiliated to Xiamen University between July 2016 and January 2017 was performed with 27+ G and 25+ G minimally invasive vitrectomy cases.The patients were divided into two groups:the first group(27+ G)received 27+ G minimally invasive vitrectomy,and the second group(25+ G)received 25+ G minimally invasive vitrectomy.The operation time,postoperative best corrected visual acuity(BCVA),intraocular pressure(IOP),postoperative comfort,incision reaction,and complications of two groups of patients were observed and compared between the two groups,including 1 day,1 week,4 week,12 week and 24 week.The sex and other data were checked by chi square test.Group operation time,postoperative BCVA,postoperative IOP were compared using t test,P<0.05 was considered statistically significant.Results:There was no significant difference between the two groups in the general conditions of age,sex,preoperative intraocular pressure and preoperative BCVA(P>0.05).All operations were successfully completed,There was no significant difference in operation time between the two groups(P>0.05).There was no significant difference in BCVA between the two groups at 1 day,1 week,4 weeks,12 weeks and 24 weeks after operation(P>0.05).The intraocular pressure of the two groups at 1 day after operation showed significant difference(P<0.05).The intraocular pressure at 1w,4w,12w and 24w after operation showed no significant difference(P>0.05).All patients in both groups had no significant pain or foreign body sensation after operation,or only mild pain,foreign body sensation,and no special discomfort.During the observation period did not appear intraocular hemorrhage,retinal holes,retinal detachment,choroidal detachment,infectious endophthalmitis and other complications.Conclusion:The 27+ G minimally invasive vitrectomy is safe and effective.The operation time is short,the injury is small and the suture is free.The postoperative inflammatory response is light and the recovery is quick.Compared with 25+ G minimally invasive vitrectomy is more minimally invasive,faster recovery,less postoperative incision leakage,low intraocular pressure,for the treatment of vitreoretinal diseases provides a new ideal surgical approach.
