Objective: The purpose of this study was,firstly,to compare the effect of traditional Chinese medicine(TCM)sandbag hot compress method with simple sandbag or traditional Chinese medicine hot compress on patients with lumbar disc herniation(LDH),to learn their different therapeutic effects,and secondly,by comparing the traditional Chinese medicine sandbag hot compress with a combination of this method with traditional Chinese medicine directional permeation therapy used in patients with LDH,the single or combined effect of traditional Chinese medicine sandbag hot compress was explored.Through our study,we wish to provide scientific evidence for the application of TCM treatment methods in patients with LDH.Method: In part Ⅰ,we compared the effect of TCM sandbag hot compress method and simple sandbag hot compress or TCM hot compress on patients with LDH.A total of 79 patients with LDH and Qi stagnation and blood stasis were recruited from Lianyungang TCM Hospital.The subjects were divided randomly into three groups: patients in Group A(n=26)were treated with sandbag hot compress,in Group B(n=26)with Chinese medicine hot compress,and in Group C(n=27)with Chinese herb sandbag combined with hot compress.All procedures were ths same in the three groups except for the hot compress method.The treatment lasted for 40 minutes each time,with 2 times a day for 2 weeks.During the intervention,14 cases dropped out and 65 cases completed the intervention.In part Ⅱ,we compared the effect of traditional Chinese medicine sandbag hot compress and a combination of this method with traditional Chinese medicine directional permeation medicine on LDH patients.A toal of 56 patients with LDH and Qi stagnation and blood stasis were recruited from Lianyungang TCM Hospital.The subjects were randomely divided into two groups: patients in Group A(n=28)received the TCM sandbag hot compress treatment,in Group B(n=28)received TCM sandbag hot compress combined with TCM directional permeation treatment.The two groups accepted the same procedure except for the treatment method.The treatment lasted for 40 minutes each time,with 2 times a day for 2 weeks.During the intervention,10 cases dropped out and 46 cases completed the intervention.Patients were evaluated with lower back pain scale from the Japanese Orthopaedic Association(JOA)before intervention,at 1 week,2 weeks,and 4 weeks after the intervention.Curative effect was assessed according to the the improvement rate of JOA score.The lower back pain was evaluated with numerical rating scale(NRS).All data were analyzed using SPSS18.0 statistic software.Results: Part Ⅰ 1 Baseline data showed no significant difference between the three groups(p>0.05).2 Comparison among groups JOA lower back pain score in group C was significantly higher than that in group A and group B at 1 week,2 weeks and 4 weeks after intervention(p<0.05).Within the score,the score in subjective symptom in group C was higher than that in the other two groups at 2 weeks after intervention(p<0.05)without difference at 1 week and 4 weeks after intervention(p>0.05);the score in clinical signs in group C were significantly higher than that in group A and group B at 1 week and 4 weeks after intervention(p<0.05),but there was no significant difference at 2 weeks after intervention(p>0.05);The score in daily activity in group C were higher than those in group A and group B at 1 week,2 weeks and 4 weeks after intervention(p<0.05).The NRS score of pain was lower in group C at 1 weeks,2 and 4 weeks after intervention than in group A and B(p<0.05).The improvement rate of JOA lower back pain score was different among the three groups after 4 weeks of intervention with a better effect in group C than groups A and B.(p<0.05).3 Intragroup comparison The scores of JOA lower back pain as well as of the three aspects within it,the subjective symptoms,clinical signs and daily activity of patients in the three groups were significantly increased at 2 and 4 weeks after intervention compared with those before intervention(p<0.05).The NRS scores of pain at 1 week,2 weeks and 4 weeks after intervention in the three groups were improved compared with those before intervention.Part Ⅱ 1 There was no significant difference in baseline data between the two groups(p > 0.05).The JOA score of lower back pain had no significant difference between A and B groups at all times points after intervention(p>0.05).Within the JOA score of lower back pain,the subjective symptom score in group B was higher than that in group A at 1 week after intervention(p<0.05)without significant difference at 2 and 4 weeks(p>0.05).There was no significant difference between the two groups in clinical signs score or in daily activity score at 1 week,2 weeks and 4 weeks after intervention(p>0.05).The NRS score of pain in group B was lower than that in group A at 1 week after intervention(p<0.05)without significant difference at week 2 and week 4(p>0.05).There was no statistical difference in the improvement rate of JOA score of lower back pain between the two groups(p>0.05).3 Intragroup comparison The scores of lower back pain and of the subjective symptoms,clinical signs and daily activity were significantly higher at the time points of 1 week,2 weeks and 4 weeks after intervention than before intervention in both groups(p<0.05).The NRS scores of pain in group A and B were improved 1 week,2 weeks and 4 weeks after intervention,compared with that before intervention(p<0.05).2 Comparison between two groupsConclusions: 1 All the t TCM sandbag hot compress,the simple sandbag or the TCM hot compress can effectively relieve the symptoms of lower back pain and improve daily activity function in patients with LDH.Comparatively speaking,the TCM sandbag hot compress scheme is more effective in relieving subjective pain and improving daily activity of patients.2 The TCM sandbag hot compress combined with TCM directional permeation therapy was superior in relieving subjective pain for a short term in LDH patients than the TCM sandbag hot compress method.
