Objective:The sensorineural deafness is a kind of clinical common,frequently-occurring and stubborn disease,whose etiology and pathogenesis is intricate.Many aspects are not clear and the treatment is especially difficult.Professor Xuan Weijun is an expert of otolaryngology department of Guangxi university of Chinese medicine,a visiting professor and researcher of the Harvard medical school of the United States and the first famous doctor of Chinese medicine of Guangxi province.Professor Xuan has made a point from the theory of the blood stasis of the liver and spleen kidney,and put his personal experience into use for the traditional Chinese medicinal follow-up treatment.The treatment of these incurable deafness often had curative effects,and his experience is quite abundant.This subject's aim is study with the teachers,so students can get some comprehensions and experiences,or they can collating medical cases,analyze and summarize the clinical treatment experience.At the same time,they can establish toxic cochlea damage animal models in gentamicin.Through the study of animal cochlea hair cells on cell morphology and molecular level,to explore the efficacy of traditional Chinese medicine(TCM)mechanism,and the Chinese medicine-Compound health ear agent's effect on the prevention and cure function of the guinea pig cochlea hair cell damage,as well as the mechanism of the cochlear hair cell damage of gentamicin toxin.It reveals the guiding significance of TCM on the entrails theory in strengthening spleen,replenishing qi and treating deafness.It also provides the reference for the prevention and treatment of the drug toxic sensorineural deafness,and provides the basis for developing and innovating the theory of treating deafness by using TCM.Methods:2.1 Clinical academic thoughts and experience summary: Exploring Professor Xuan's clinical thinking and dialectical method.Professor Xuan's experience of diagnose and treatment of sensorineural deafness are summarized systematically,and we summarized Xuan's commonly used medicinal herbs and prescriptions to find his law of drug use.2.2 Animal experimental research: 60 healthy adult guinea pigs were selected,whose auricle reflex was sensitive and weight was 300-350 g.A total of 60,randomly divided into the following blank group,gentamicin(GM)group and traditional Chinese medicine + gentamicin group(traditional Chinese medicine(TCM)+ GM group),20 animals in each group.The experiment was set a time for 21 days,and the groups were starting and ending at the same time.Observation indexes :(1)Observing and comparing the morphological changes of cochlea hair cells in different groups by using cochlear stretched preparation technique,and conducting statistical treatment.(2)Using RT-PCR molecular biology technology to observe the changes of cochlea SOD.(3)Testing the blood content of BUN and Cr.(4)Doing pathological section of the kidney.Results:Guinea pig's ABR detection results: GM group's hearing threshold of5,7,9 days were higher than other two groups(P < 0.05),Chinese medicine +GM group compared with the blank group,although they had differences,the differences are not as significant as GM group compared with the blank group.The guinea pig cochlea morphology observation results: GM group's outer hair cells were damaged the most,Chinese traditional medicine + GM group were damaged lighter.The total amount of the damaged cells of these two groups had significant differences(P < 0.05).The content of SOD1 in cochlea detection shows: there was no significant differences between the blank group and the GM group,but there was a statistical difference among the blank group,the GM group and the TCM +GM group(P<0.05).Renal function examination: there were significant differences in urea nitrogen and creatinine compared with the other two groups on the ninth day(P < 0.05).The renal tissue morphology observation: the damage degree of kidney tissue in GM group was significantly serious than that in the other two groups,and the TCM+GM group was damaged lighter.Conclusion:Compound healthy ears agent can reduce the ototoxicity of GM.it can obviously improving c AMP,adjusting the nucleic acid metabolism,scavenging free radicals and so on,the pharmacological effect is considered possibly as the main reason of the antagonism effect on GM ears and renal toxicity.Its function of protecting renal is conducive to the GM discharge,and reduce the GM accumulate in the body,which is an important aspect of ease GM ototoxicity.
