Prevalence Investigation Of 200 Farmers' Dermatitis/Eczema And Urticaria In Rushan City

BackgroundAtopic dermatitis(AD)is a kind of dermatitis associated with hereditary allergic constitution.Besides the performance of lesions which are symmetrical,exuding and recurring,patients usually have a personal or family history of asthma,allergic rhinitis or conjunctivitis with higher levels of IgE or ESO in serum.The pathogen is associated with genetic susceptibility,exposure to air allergens,autoantigen,infection,damage of skin barrier function,and so forth.The prevalence has a positive correlation with the degree of local urbanization and industrialization.According to previous studies,the global prevalence of AD is 15%-20%in children and 1%-3%in adults,and it has widely influences on quality of life.Urticaria is a series of common allergic diseases caused by expansion and higher permeability of small vessels in skin or mucosa,which lead to local edema.It's related to food or drug allergies,infections,cold,heat,light,physical irritation,insect bites,mental disorder,endocrine changes,and even some are related to heredity,immunity and tumor,while some are uncertain.The prevalence of chronic spontaneous urticaria is 0.1%-0.3%in European and American countries.Due to its long disease course,CUS usually has severe impacts on patients' quality of life.While in our country the investigations of AD in the past were focus on children in local areas,seldom related to adults,and researches of eczema were located too.The occurrence of these two diseases are influenced by environment and exposure,therefore,it may be so different between urban and rural areas.Since the conditions of diseases in rural areas may be different,it's necessary to conduct targeted investigations.ObjectiveTo investigate the prevalence situation of 200 farmers' dermatitis/eczema,including AD,and urticaria in Rushan city.MethodsTaking the Questionnaire on Epidemiological Investigation of Chinese Dermatitis/Eczema and Urticaria led by Professor Zhang Jianzhong of Peking University Peopl e's hospital as a template.Randomly investigating 200 farmers who are having a re st in the street from Zhaominzhuang Village,Shentongmiao Village and Maling Vill age of Rushan city.Investigating the prevalence situation of dermatitis/eczema,espec ially AD,and urticaria by learning the skin health status of respondents,consisting of ratio of diseases during the life,situation of diseases,impacts of life,and treatm ent status.The diagnosis of AD using Zhang Criteria.The data analysis using Micr osoft Excel.The ratio of disease using percentage.The analysis of influences using OR and 95%CI.The situation of diseases,impacts of life,and treatment status usi ng direct description.Results1.Gender and age:In the 200 respondents,there were 96 males(48%),104 female(52%);the average age was 58.3 years old.2.Ratio of diseases:There were 23 patients(11.5%)in dermatitis/eczema group,4 patients(2%)of AD were certainly diagnosed,8 patients(4%)had suspected diagnosis as AD,6 patients(3%)had contact dermatitis,5 patients(2.5%)had other dermatitis/eczema,the ratio of dermatitis/eczema was 11.5%,and AD was 2%-6%.There were 59 patients in urticaria group,33 patients(16.5%)had acute spontaneous urticaria,15 patients(7.5%)had contact urticaria,7 patients(3.5%)had cold urticaria,5 patients(2.5%)had dermographism,4 patients(2%)had chronic spontaneous urticaria,the ratio of urticaria was 29.5%(some patients had two or kinds of urticaria or more).3.Situation of diseases:More life pressure was reported in dermatitis/eczema(OR=2.72,95%Cl[1.06,7.01]).Dermatitis/eczema more likely occurred in summer(17 patients,73.9%of all);season shifting,torridity and humidity were common risk factors;eating fish,shrimp,crab,or spicy food may be other aggravating factors;lesions mostly appeared at extensor shins and forearm.Drinking tea may be associated with urticaria(OR=3.67,95%CI[1.56,8.68])and allergic rhinitis was significantly associated with urticaria(OR=2.99,95%CI[1.15,7.80]).Some patients of Urticaria would aggravate after eating some certain foods,such as pupa,seafood,larva of cicada and locust;some were related to cold,scratching,contact of botany(peanut vine,com leaf or flowers)and solarization;in the recurrent ones,their urticaria more likely attacked at summer or at night.4.Impacts and treatment status:47.8%dermatitis/urticaria patients' life was influenced and the crowd impact rate was 5.5%.Visiting rate of dermatitis/urticaria was 26.1%.32.2%urticaria patients,life was influenced and the crowd life impact rate was 9.5%.Visiting rate of urticaria was 20.3%.The crowd impact rate refers to the proportion of all interviewees whose life was influenced by diseases.Conclusion1.Compared to previous reports,ratio of AD is lower but ratio of urticaria is higher in local farmers.Acute spontaneous urticaria,contact urticaria and cold urticaria are common types.2.Local farmers' dermatitis/eczema is associated with pressure and urticaria is associated with drinking some special teas.3.Dermatitis/eczema and urticaria have some impacts on local farmers' life,but the visiting rate of patients is low.
