ObjectiveAlong with the development and application of minimally invasive techniques in the department of thoracic surgery,The great value of thoracoscopy in the diagnosis and treatment of various thoracic diseases is already evident.In recent years,as a new endoscopic technology,Uni-port Video-assisted Thoracoscopic Surgery is favored by the vast majority of thoracic surgeons and patients because of the satisfying results in the surgical treatment of non-complex diseases of department of thoracic surgery,such as pneumothorax and small lesion lung cancer.As a new trend,the appearance and application of it promote the development of minimally invasive department of thoracic surgery.However,it has rarely been reported in surgical treatment of mediastinal mass.In this study,through comparative analysis,we discussed the perioperative efficacy of Uni-port Thoracoscopic Surgery for mediastinal tumor resection,and summarized the advantages and disadvantages of this new minimally invasive endoscopic surgery.MethodThe medical files of 163 inpatient who under examination and resection of the mediastinal tumor by video-assisted thoracoscopic surgery in Department of thoracic surgery,the First Affiliated Hospital of Dalian Medical University from October 2015 to September 2017 was collect.Exclude 2 cases which transfer to thoracotomy and 5 cases which under simultaneous bilateral thoracoscopic surgery.In the rest of the patients,45 cases were under resection of mediastinal tumor by Uni-portal Thoracoscopic Surgery(UP-VATS),77 cases were under resection of mediastinal tumor by Single utility port Video-assisted Thoracoscopic Surgery(SP-VATS),34 cases were under resection of mediastinal tumor by conventional three port Video-assisted Thoracoscopic Surgery(3-port VATS),A retrospective analysis of the medical files of three groups,Comparison of the differences was made between the three groups of data in the tumor location,tumor size,operative time,intraoperative blood loss,first day postoperative drainage volume,total postoperative drainage volume,postoperative drainage tube indwelling time,postoperative hospital stay,postoperative analgesic application and postoperative complications.ResultsNo perioperative deaths were found in three groups of patients,There were no statistically significant differences between three groups in patients ' age,gender,mediastinal tumor location,mediastinal tumor size,and postoperative complications(p>0.05).And there is no difference between any two groups.There were significant statistical differences between three groups in the operative time,intraoperative blood loss,first day postoperative drainage volume,total postoperative drainage volume,postoperative drainage tube indwelling time,postoperative analgesic application and postoperative hospital stay(p<0.05)?ConclusionOn the basis of comparing the traditional thoracotomy with the absolute advantage and the substitution,in the resection of mediastinal tumors,Uni-portal Thoracoscopic Surgery minimizes the chest wall trauma of surgical approach to the largest extent.,compared with the SP-VATS and the traditional 3-port VATS,it is less trauma,lighter pain,faster recovery,and is a more minimally invasive but also a safe and very effective way of operation,It is of great clinical significance for the diagnosis and treatment of mediastinal tumors,which can obviously reduce postoperative pain,accelerate postoperative rehabilitation and improve the quality of life after operation.it's worthy of clinical application.
