Objective To observe the intervention effect of stimulating Zigong(EX-CA1)combined with remote acupoints or simple remote acupoints by Transcutanclus Electrical Acupoint Stimulation(TEAS)on Primary Dysmenorrhea(PD).And to compare the clinical efficacy of different TEAS stimulation treatments on patients with Primary Dysmenorrhea(PD).Methods 78 patients, diagnosed as primary dysmenorrhea, met the inclusive criteria, were randomly divided into three groups: blank group(group A),simple remote acupoints group(group B)and Zigong(EX-CA1)combined with remote acupoints group(group C).And 26 patients in each group. The group A did not receive any treatment. In group B,the points selected were bilateral Sanyinjiao(SP6)and Diji(SP8).While bilateral Zigong(EX-CA1),left Sanyinjiao(SP6)and Diji(SP8)were taken in group C.Four pairs of acupoints in group B and C were stimulated by TEAS at a frequency of 2/100 Hz and the degree was based on patient comfort.Patients were treated on the day of menstruation, each time for 30 minutes,1 time a day,for 5 times. And the treatment lasted for 3 menstrual cycles. The therapeutic result was observed by assessing the Cox Menstrual Symptom Scale(CMSS).Three months after the end of treatment, the CMSS was used to assessing the long-term efficacy of group B and group C.Results Curative effect evaluation: The total effective rate of group A,B and C were 12.50%,69.57% and 87.50%.The clinical efficacy of group B and group C is obviously better than that of group A(P<0.05),but no significant difference between the two groups(P>0.0167).During the treatment and follow-up,the cramps severity, cramps duration, scores of CMSS severity and scores of CMSS duration in groups A and B were significantly different from those before the intervention(P<0.05).Cramps severity evaluation: After the first and second menstrual cycles, the degree of cramps in group C was significantly lower than group A,the difference was statistically significant(P<0.05).There was no significant difference between the other two groups.Three menstrual cycles later, the degree of cramps in groups B and C were significantly lower than group A(P<0.05),and group A was more effective than group B(P<0.05).Cramps duration evaluation: During the treatment period, the cramps duration of groups A and B was more significantly shorter than group C(P<0.05).But there was no significant difference between group B and C(P>0.05).CMSS severity evaluation: After the first menstrual cycles, scores of CMSS severity in group C was significantly lower than group A,the difference was statistically significant(P<0.05).There was no significant difference between each groups. After the second and third menstrual cycles, scores of CMSS severity in groups B and C were significantly lower than group A(P<0.05).But there was no significant difference between group B and C(P>0.05).CMSS duration evaluation: After the first menstrual cycles, there was no significant difference between the three groups(P>0.05).After the second and third menstrual cycles, scores of CMSS duration in groups B and C were significantly lower than group A(P<0.05).But there was no significant difference between group B and C(P>0.05).Three months after the end of treatment, all the above observations in groups B and C have rebounded to varying degrees, and no significant difference between the tow groups(P>0.05).Conclusion The two TEAS groups can relieve the severity and shorten the duration of some clinical symptoms of PD.The two groups had similar curative effects, but TEAS on Zigong(EX-CA1)combined with remote acupoints worked faster and the clinical efficacy tended to be better. The short-term efficacy of TEAS in relieving dysmenorrhea symptoms and shortening the duration of dysmenorrhea is obvious, but the long-term efficacy is not evident.
