Purpose:To analyze the accuracy of various auxiliary examinations for measuring the diameter of ascending aortic aneurysm.Methods:This research is a clinical randomized controlled trial.Screening for 20 patients with ascending aortic aneurysm operatedat the cardiac surgery of the First Affiliated Hospital of Xiamen University from June 2016 to May 2017(since the maximum diameter of the aortic aneurysm can be measured directly during the operation with a sterilized caliper,So excluded aortic arch aneurysm anddescending aortic aneurysm).Patients were divided into A,B,C,D four groups.In group A,transthoracic color Doppler examination,measured the maximum diameter of aneurysm bulging.In group B,computed tomography angiography(CTA)and three-dimensional reconstruction were performed.The maximum diameter of the aneurysm was measured at the site of the largest site.In group C,aortic angiography was performed before surgery,and the maximum diameter of the aortic aneurysm was accurately measured during the operation.In group D,patients underwent ascending aorta replacement under cardiopulmonary bypass and the aneurysmwere fully exposed during the operation.The largest diameter of the aortic aneurysm was measured with a sterilized vernier caliper at the point where the aneurysmbulged.Apply SPSS 18.0 statistical software.The data measured by group A,group B,and group C were compared with the data of group D.The statistical data were expressed as the mean±standard deviation,and the results were compared using the one-way analysis of variance(A,B,C and D).The longest diameter of the aortic aneurysm in the four groups.The LSD-t test was used to compare each group of A,B,C,and D.The difference was considered statistically significant at P<0.05,was considered statistically significant to analyze the accuracy of various auxiliary examination results.RESULTS:All the patients enrolled in this study were performed transthoracic color Doppler ultrasound,computed tomography angiography(CTA),aortic angiography(DSA),and aortic aneurysm measurement using vernier calipers during ascending aorta replacement surgery.Among them,the auxiliary examination and intraoperative findings:10 patients were true aortic aneurysm in 20 patients,4 cases were pseudoaneurysm;4 cases were aortic aneurysm with ascending aortic dissection,4 patients showed significant rupture.It can be seen that for the diagnosis of true aneurysms,pseudoaneurysm,and dissecting aneurysms,the diagnosis of various auxiliary examinations is exactly the same.Transthoracic color Doppler and computed tomography angiography(CTA)revealed that 9 intra-aortic intra-articular hematoma were detected in the aortic aneurysm,while it was found in 6 patients and pericardial defect 1 during aortic replacement surgery,one case was invading the aortic valve and two cases had coronary sinus dilation.The maximum diameter ofthe aortic aneurysm(DU-max)measured by color Doppler ultrasound:64.860±10.839;Computed tomography angiography was 64.614±10.184mm;The DSA measured was 64.242±10.831mm.The maximum diameter of the tumor measured during ascending aortic replacement(measured during systole):OPERATION-MAX:64.225 ± 10.425 mm.Conclusion:Transthoracic color Doppler,computed tomography angiography(CTA)and digital subtraction angiography can be used as clinical diagnostic criteria for aortic aneurysm and measurement of aortic aneurysm diameter,but the measurement accuracy of transthoracic color Doppleris the significant.
