Diabetic nephropathy is the most serious complication of diabetes.It can cause renal failure through interstitial and tubular fibrosis and eventually progresses to end-stage renal disease.The previous medical community believes that patients with diabetic nephropathy and other chronic kidney disease are more likely to die of cardiovascular disease than ESRD,but according to a foreign country,a retrospective study on the treatment and long-term follow-up of patients with type 2 diabetes has been followed up for 2.8 years.After tracking and collecting and analyzing 3,228 adult patients with Type ? diabetes in the world's largest diabetes patient clinical database,it was concluded that in diabetic patients,death due to ESRD is actually more than death from cardiovascular disease.It is common.About 19.5%of these patients die because of end-stage renal disease,which is about 2.5 times the proportion of cardiovascular deaths,and 1.5 times the all-cause mortality,which is about 2.5 times.In addition,ESRD is not only common in patients with T2DM,it is also the main cause of death for type ? diabetes,and the incidence of T1DM is also increasing rapidly at a rate of 2%to 3%per year worldwide.It can be seen that the clinical treatment of ESRD has become a major global health issue.Previous conventional hemodialysis,insulin application and related symptomatic treatment have great limitations in controlling the occurrence and development of complications of diabetic nephropathy,and can no longer meet the needs of current treatment expectations.With the rapid development of organ transplantation science in recent years,the application of this new technology of islet-kidney combined transplantation gradually replaces the clinical pancreas-kidney combined transplantation with its unique advantages.It is diabetic nephropathy,especially the treatment of ESRD has expanded a new path.Among these,xenotransplantation plays an important role.This article will review the kidney-islet joint transplantation from historical review,donor selection,donor preparation,and immune support.
