Object The aim of this study was to detect the expression of mast cell tryptase and substance P(SP)in intestinal mucosa of patients with irritable bowel syndrome by immunohistochemistry,and investigate the relationship between its expression and the clinical symptoms of IBS patients.Methods From July 2013 to November 2017,there were 29 cases of IBS patients in our department of the Second Affiliated Hospital of Soochow University included in the experimental group(IBS group)according to the standard of Roman III.16 healthy people for physical examination were received endoscopic examination with negative result as normal control group(group N).After analysis of pathological diagnosis for biopsy of the intestinal mucosa in rectosigmoid iunctions,the expression of mast cell tryptase and SP were detected using immunohistochemical staining.Finally,the relationship between its expression and the clinical symptoms of IBS patients was performed by appropriate statistical methods.Results 1.The positive expression score of mast cell tryptase intestinal mucosa was mainly located in the stroma.the IBS group were 5.86±2.65,and the group N were 4.10±1.58.The activation level of MC was significantly higher in intestinal mucosa of IBS patients than that in the group N(P=0.008<0.05).The positive expression score of mast cell tryptase in the IBS subtypes: IBS-D group 6.0±2.7,IBS-C group 5.6±2.5,and IBS-M group 6.0±2.9;by the analysis of variance,there was no significant different activation level of MC in three IBS subtypes(P=0.102>0.05).The positive expression score of mast cell tryptase between men and women in the IBS group: male group 5.73±2.69,female group 5.91±2.62,there was no significant different activation level of MC between men and women in the IBS group(P>0.05).2.The positive expression score of SP in intestinal mucosa was mainly located in the nucleus and cytoplasm.the IBS group were 0.2(0.1,1.1),and the group N were 0.1(0,0.7).there was no significant different expression of SP between IBS patients and normal healthy people(P=0.224>0.05).The expression of SP in the IBS subtypes: IBS-D group 0(0,1),IBS-C group 0(0,1),and IBS-M group 0(0,1.75),by the Analysis of Kruskal-Wallis,there was no significant different expression of SP in three IBS subtypes(P=0.77>0.05).The expression of SP between men and women in the IBS group :male group 0.4(0.1,1.3),female group 0.2(0.05,1.1),there was no significant different expression of MC between men and women in the IBS group(P>0.05).3.The relationship of the expression of mast cell tryptase and SP:by the Spearman correlation analysis,in the group IBS,The activation level of MC and SP was no statistical significance(r=-0.06,P=0.77>0.05).and there was no statistical significance in the group N and the IBS subtypes.Conclusion 1.The activation level of MC were significantly higher in intestinal mucosa of IBS patients than that in normal healthy people;There was no significant different activation level of MC in three IBS subtypes and no significant different activation level of MC between men and women in the IBS group either.2.There was no significant different in the expression of SP in intestinal mucosa between IBS patients and normal healthy people.There was no significant different expression of SP in three IBS subtypes,and no significant different expression of SP between men and women in the IBS group either.3.There was no statistical significance of the activation level of MC and the expression of SP in the group IBS,and no statistical significance in the group N and the IBS subtypes.
