Abstract/Summary:
Objective: To analysis the incidence and risk factors for Delphian lymph node metastasis in papillary thyroid carcinomaMethods: A retrospective analysis of clinical data of 32 patients with primary PTC information from the first hospital affiliated to Soochow University general surgery.All the operations were finished by the same treatment team between April 2017 and April 2018,and these 32 patients are confirmed by the intraoperative and postoperative pathologic diagnosis.The central lymph nodes of 32 patients are divided into two subregions:Delphian lymph node and central lymph node.We name the Delphian lymph node for ‘Delphian lymph node”,and name the central lymph node except the Delphian lymph node for ”The central lymph node”.To analysis the correlation of lymph node metastasis with patients’ age,sex,tumor diameter,multifocal or not,the capsule invasion,the central lymph node metastasis.Results: All of the 32 patients,13 patients with Delphian lymph node metastasis and 19 patients without.In the operation,the surgeon dissect 63 Delphian lymph nodes 1.73 per patient on average,and 14 were proved metastatic,the rate is 22.2%(14/63).The factors of Delphian lymph node metastasis include tumor size over 1 cm(P=0.03)、multiple lesions(P = 0.006)and the central lymph node metastasis(p=0.005).Conclusion: The central lymph node dissection is of great significance for patients with thyroid papillary carcinoma.As an important part of the central lymph node,The Delphian lymph has a high rate of metastasis.The tumor size,multiple lesions and he central lymph node metastasis may be independent risk factors for the Delphian lymph node metastasis,patients with these factors,surgical exploration of the Delphian lymph node should be positive and careful.The definitive diagnosis of the Delphian lymph nodes can help the surgeon take an active surgical strategy,and guides more active postoperative follow-up.
