Outcomes Of Balloon Dilation Eustachian Tuboplasty Combining Tympanotomy Tube Insertion In The Treatment Of Refractory Secretory Otitis Media

Objective:To investigate the efficacy of balloon dilation eustachian tuboplasty(BDET)combined with tympanotomy tube insertion(TTI)to improve the outcome of refractory secretory otitis media(RSOM).Methods:Twenty-six patients(40 ears)who underwent BDET combined with TTI at our institute between January,2017 and June,2017 were included in this study.Eustachian tube score(ETS),pure tone audiogram(PTA),and air bone gap(ABG)were performed before and 1 mo,3 mo,6 mo after surgery in each patient.Paired student t test was utilized to the compare the results obtained before and after surgery.Results:There were significant differences in ETS,PTA,and ABG in patients with 1,3,6 months after operation.ETS obtained after surgery(5.18±1.13,P=0.000;5.93±0.92,P=0.000;6.58± 1.15,P=0.000)were significantly higher than that obtained before surgery(2.08±0.53),with an increasing tendency over time,indicating the gradual recovery of the function of eustachian tube.PTA obtained after surgery(29.9±6.93,P=0.000;23.4±5.36,P=0.000;17.6±3.20,P=0.000)were significantly reducer than that obtained before surgery(36.9±4.63),with an decreasing tendency over time,indicating the hearing is getting better.ABG obtained after surgery(6.88±2.45,P=0.000;6.13±2.12,P=0.000;5.38± 1.33,P=0.000)were significantly reducer than that obtained before surgery(28.3±4.74),with an decreasing tendency over time,indicating the hearing is getting better too.Tympanograms of 8 ears that have been removed from the tube have been converted to type "A".Conclusion:ETS deserves to be used in clinical practice.The operation of BDET combined with TTI is simple and safe,it can effectively improve the eustachian tube function of RSOM,relieve symptoms,improve hearing,After 6 months of operation,the treatment effect is better and better,and the symptoms are improved to be stable and have good short-term clinical treatment effect.
