Objective:Diabetes has now become a global public health issue,with a wide range of prevalence and a rapid increase in prevalence.According to Global report on diabetes by World Health Organization in 2016,the amount of diabetic patients has risen from 108 million in 1980 to 422 million in 2014,as well as the prevalence rate from 4.7% to 8.5%.The prevalence of diabetes in Chinese adults is 10.9%.Shanghai,as the biggest city in China,shows a rapid increase in the prevalence of diabetes.Shanghai Municipal Health Bureau announced on November 12,2006 that the prevalence of diabetes in Shanghai was 8.6%,compared with 1.01% in 1980,the most recent data from CDC was 17.6% among people with diabetes over the age of 35.These data document a rapid increase in diabetes in the shanghai population.Inheritance?behavior and life style contribute to the development of diabetes.In our study,we analyze the prevalence and control of diabetes in suburb,subururban and urban as well as immigrants and study the risk factors of diabetes and related metabolic disorders and then explore how to prevent and control diabetes.We hope to Provide epidemiological basis for the prevention and control of diabetes in Shanghai.Methods: Our research is an cross-sectional study,the objects are from the key project of Three Year Action plan for Public Health of Shanghai Municipal Health and Family Planning Commission?Screening and evaluation system of autoimmune thyroid disease(AITD)in adults in Shanghai?.We chose ten communities both in urban,suburb and suburban areas of Shanghai,then 10 different neighborhood committees in every community chose 4024 people in total as our objects.In our research we excluded people with any diseases that may affect glucose tolerance,pregnant women,people with hepatitis?tuberculosis?HIV or infectious diseases?bone fracture?severe mental disease.During the our research,we obtained the basic information,including name,sex,place of birth,residence in Shanghai,occupation,education,income,current medication,and height,weight,waist circumference,blood lipid levels,fasting venous plasma glucose,glucose 2h after OGTT,glycosylated hemoglobin,uric acid.Results:1.According to the ADA 2010 criteria,the prevalence of diabetes among the 4024 subjects in Shanghai community was 8.5%,among male was 9.6% and among female was 7.9%,The prevalence of prediabetes was 34.3%,among male was 36.4% and among female was 32.6%.The rate of awareness treatment and control of diabetes in the general population were 57.0%,47.1%,50.3% respectively,among male were 53.4%,44.4% and 47.4% respectively,among female were 60.8%,50.0% and 53.5% respectively.The prevalence of diabetes in suburb,suburban and urban were 8.0%,9.9% and 7.7% and the prevalence of prediabetes were 32.0%,33.5% and 37.3% respectively.The rate of awareness and control of diabetes in suburbs were lower compared with urban and suburban.There was a similar rate of awareness among the immigrants and the local people,but the local people have a significantly better control of diabetes.2.sex,older age,family history overweight,obesity,central obesity,elevated systolic blood pressure,current cigarette smoking,living in suburban,low occupational classes and elevated serum triglyceride levels were all significantly associated with a higher risk of diabetes.Higher serum HDL-cholesterol level was associated with a lower risk of diabetes.In addition,older age,overweight,obesity,central obesity elevated systolic blood pressure,low occupational classes,elevated serum LDL-cholesterol and Hyperuricemia were positively associated with a higher risk of prediabetes.Higher serum HDL-cholesterol was associated with prediabetes.Conclusion:The prevalence of diabetes aged 18-65 years in Shanghai was high.Among th e three regions,the prevalence of diabetes in suburban areas was higher while the rate of awareness and control of diabetes in suburbs was lower.Compared with the immigrants,local people have a better control of diabetes.We can analyze the epide miological characteristics of diabetes in Shanghai,then we can prevent and control t he diabetes according to different risk factors as soon as possible and reduce the m edical insurance burden caused by diabetes.
