Comparison Of Effects Of The Follow-up Treatments After Insulin Pump Intensive Treatment On Newly Diagnosed Patients With Type 2 Diabetes

Objective:To observe the curative effect of different follow-up treatment regimens after initial intensive treatment of type 2 diabetes mellitus with insulin pump.Methods:A total of 150 patients with type 2 diabetes mellitus were selected from Jiangning Hospital of Nanjing from March 2015 to July 2016,and were randomly divided into three groups(A,B and C)with an average age of 50 in each group.Patients in Group A were treated with oral hypoglycemic drugs,Group B with basal insulin plus oral hypoglycernic drugs and Group C was premixed insulin analogue plus oral hypoglycemic drugs.The levels of fasting blood glucose(FBG),postprandial blood glucose(P2hBG),insulin,C-peptide and HbAlc were measured before treatment,3 months,6 months and 12 months after treatment,The index of pancreatic p-cell function(HOMA-p),index of insulin resistance(HOMA-IR)were calculated.At the same time,the incidence of hypoglycemia and body mass index(BMI)were also observed.Results:(1)The levels of FBG,P2hBG and HbAlc in three groups of type 2 diabetic patients after 3 months,6 months and 12 months of treatment decreased gradually.After 12 month treatment,FBG and P2hBG were lower than those before treatment,the difference was statistically significant,P values<0.05;HbAlc levels were no significant difference,P values>0.05.(2)The fasting C-peptide level increased and HOMA-IR index decreased at 3 months,6 months and 12 months after treatment in three groups of type 2 diabetic patients,but the differences were not statistically significant,P values>0.05.Compared with the pre-treatment,the HOMA-p index increased gradually at 3 months,6 months and 12 months in three groups of type 2 diabetic patients after follow-up treatment,and increased significantly at 6 and 12 months,the difference was statistically significant,P values<0.05.At 12 months,the HOMA-p index of group B was significantly higher than that of group A and C,the differences was statistically significant,P values<0.05.(3)BMI value of three groups slightly increased at 3 months,6 months and 12 months after treatment compared with those before treatment,but the differences were not statistically significant,P values>0.05.The incidence of hypoglycemia of group B was lower than that of group A and group C,the differences were statistically significant,P values<0.05.Conclusions:For patients with newly diagnosed type 2 diabetes after insulin pump intensive treatment,the best program of follow-up treatment is the basic insulin injection plus oral hypoglycemic drugs,which not only can control blood sugar levels of patients with type 2 diabetes,but also reduce the incidence of hypoglycemia,not increase the patient's body weight simultaneously.
