Evaluation The Efficacy Of Laparoscopic Splenectomy With Devascularization And Open Surgery Treatment Of Portal Hypertension

Objective: To investigate the treatment efficacy of Laparoscopic splenectomy with devascularization(LSD)and Open surgery splenectomy with devascularization(OSD)in patients with portal hypertension.Then to search for a type of surgery,which may bring for better efficacy,fewer postoperative complications,effective treatment of portal hypertension and improve the quality of life..Methods: To collective 40 cases of portal hypertension,who received EDS from December 2012 to December 2016 in our hospital.The cases were divided into two groups by LSD and OSD.To statistic and analyze the perioperative parameters,the postoperative portal venous thrombosis(PVT),the changes of postoperative platelet(PLT),the postoperative prothrombin time(PT),the postoperative exhaust time,the beginning eating time of postoperative,the hospital day,postoperative recovery and complications of patients by retrospective analysis.To find out the differences of treatment between the two type of surgery and analyze the causes,so as to find a more effective and more suitable operation method.Results: The patients were divided into laparoscopic group(group A,20 cases)and Open operation group(group B,20 cases).There were no significant differences of the general situation,such as age,gender,preoperative liver function classification,spleen size,the changes of postoperative platelet and the postoperative prothrombin time(PT),between the two groups(P>0.05).There were significant differences of the perioperative condition,the postoperative portal venous thrombosis(PVT),postoperative exhaust time,the beginning eating time of postoperative,the hospital day and the postoperative complications,such as abdominal effusion,incision infection,pulmonary infection and pancreatic fistula,between the two groups(P<0.05).Conclusion: Portal hypertension and a series of pathophysiological changes caused by it(splenomegaly,esophageal varices,ascites,etc.)has been a hot research and difficult treatment at home and abroad.Through this study,we believe that LSD was more effectively than other operation for treatment of portal hypertension and caused clinical syndrome,but could reduce the postoperative complications,improve the quality of living and quality of life of patients as well.Compared with the traditional open surgery,laparoscopic surgery had advantages of minimally invasive,less injury and quick recovery.In the condition of a good endoscopic technology,LSD is an effective surgical treatment of portal hypertension.
