ObjectiveTo investigate the expression and significance of TLR4 on peripheral blood T lymphocyte subsets in patients with SLE(systemic lupus erythematosus).Method According to the American College of Rheumatology classification criteria for SLE(1997),we selected 49 patients with SLE and 19 cases of normal healthy persons,their clinical data had also been collected.According to the systemic lupus erythematosus disease activity,patients with SLE were divided into two groups,which include active group(SLEDAI is equal to or more than 5)and stable group(SLEDAI<5 points).By using flow cytometry,we detected 31 cases of SLE group,18 cases in stable group and 19 cases of healthy control peripheral blood T lymphocyte subsets had percentage of TLR4 surface.We compared the TLR4 percentage of peripheral blood T cell subsets in the peripheral blood of the active group,stable group and healthy control group,and we also analyzed their significance.SPSS 17 statistical software was used to analyze the correlation between TLR4 percentage of T lymphocyte subsets and clinical manifestation and laboratory indexes in peripheral blood SLE lymphocyte subsets.Results(1)The percentage of CD8~+T lymphocytes in the active group(33.05±7.88)and stable group(38.16±12.43)%were higher than those in healthy control group(25.51±4.19)%(P<0.01).There was no significant difference between the active group and the stable group.The percentage of%CD4~+T lymphocytes in healthy control group(36.31±3.79)was higher than that in active group(31.4±9.14)%(P<0.05).There had no significant difference between each of healthy control group,stable group and active group.(2)TheCD4~+T lymphocyte percentage of Anti Po antibody positive group(34.39±9.49)was higher than anti Po antibody negative group(29.50±9.10)%(P=0.044),The CD4~+T lymphocyte percentage in other indicators of the laboratory had no difference between positive group and negative group.The percentage of CD8~+T cells was negatively correlated with CRP(r=-0.363,P=0.010).There was no significant correlation between the percentage of CD4~+T cells and any of laboratory indexes.(3)The TLR4 percentage of SLE peripheral blood CD4~+,CD8~+T lymphocyte cell surface:the TLR4~+CD8~+T lymphocyte percentage ofgroup activities(4.60±2.40%)and stable group(4.85±1.86)%were higher than that in the healthy control group(292±1.12%)(P<0.01),the difference had no statistical significance in active group and the stable group.The TLR4~+CD4~+T lymphocyte percentage of Activity group(4.72±2.24)%and stable group(4.80±2.35)%were higher than the healthy control group(3.01±1.29)%(P<0.01),there was no significant difference in activity group and stable group.(4)Clinical manifestations:The TLR4 percentage in CD4~+T lymphocytes of in Raynaud's phenomenon group(5.36±2.51%)was higher than that without Raynaud's phenomenon group(3.87±1.48%)(P=0.032);The TLR4 percentage of neuropathy group(4.87±2.23)%was higher than that without neuropathy group(1.98±0.18%)(P=0.034).There were no statistically significant differences between the positive group and control group with other clinical manifestations.Talking all clinical manifestations,the percentage of TLR4 lymphocytes CD8~+T had no statistically significant differences between the positive and negative groups.(5)Indicators of the laboratory examination,the TLR4~+CD8~+T lymphocyte percentage of leukocyte reduction group(5.21±2.15%)was higher than that of group without leukocyte reduction(3.64±2.23%)(P=0.004),The TLR4~+CD4~+T lymphocyte percentage(5.19±2.20%)respectivelyhigher than the group without leukocyte reduction(3.84±2.15%)(P=0.008).(6)The percentage of TLR8~+CD4~+T lymphocytes was negatively correlated with thecomplementC4.(r=-0.506,P=0.000);ThepercentageofTLR4+CD4+T lymphocyteswasnegativelycorrelatedwiththecomplementC4(r=-0.561,P=0.000).The percentage of TLR4~+CD4~+T cells was positively correlated with the anti dsDNA(r=0.306,P=0.048).The percentage of TLR4~+CD8~+T cells was positively correlated with the anti dsDNA(r=0.409,P=0.007).The percentage of TLR4~+CD8~+T lymphocytes and TLR4~+CD4~+T lymphocytes had no correlation with CRP,ESR,C3,24h urinary protein quantitative,ANA quantitative.Conclusions(1)The percentage of CD8~+T cells in systemic lupus erythematosus activity was higher than that in normal persons,the percentage of CD4~+T cells was lower than that in normal persons.(2)the TLR4 expression of CD8~+CD4~+T lymphocytes in patients with systemic lupus erythematosus was higher than that in normal controls.(3)The percentage of TLR4~+CD8~+T cells and TLR4~+CD4~+T cells in systemic lupus erythematosus was negatively correlated with C4 and positively correlated with anti dsDNA.(4)The percentage of TLR4 in peripheral blood T lymphocytes in patients with systemic lupus erythematosus was related to the SLE condition,and the percentage of TLR4 was detected as a reference index for diagnosis and diagnosis of lupus activity.
