Objective:To investigate the relationship of betacellulin(BTC)exon1TGC19GGC gene polymorphism with type 2 diabetes and diabetic nephropathy.Methods:268 cases of type 2 diabetes patients and 160 cases of healthy control group were recruited in the study.The type 2 diabetes was divided into A?B and C groups based on glomerular filtration rate(GFR),GFR?90mL/(min·1.73 m~2)was A group,90ml/(min·1.73 m~2)>GFR?60ml/(min·1.73 m~2)was B group,GFR<60ml/(min·1.73 m~2)was C group.At the same time,the type 2 diabetes patients were divided into“non-diabetic nephropathy patients”group(AER<30mg/24h)and“diabetic nephropathy”group(AER?30mg/24h)based on urinary albumin excretion rate(AER).The subjects in each group were extracted venous blood,their blood specimens were extracted genomic DNA.To screen betacellulin exon1TGC19GGC gene polymorphism of type 2 diabetes and normal control group with polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and to compare the genotype and allele.Clinical datas of subjects with different genotypes were analyzed and compared.Results:(1)TG genotype and G allele frequencies were no significant difference between the control group and type 2 diabetes group(P>0.05),there were no difference in A?B and C groups(P>0.05),there were no difference between diabetic nephropathy and non diabetic nephropathy also(P>0.05).(2)Homa2-%?in the TG genotype less than TT genotype(P<0.05),There were no difference in?C30/?G30,?C2h/?G2h,Homa2-IR,Homa2-%S between TT genotype and TG genotype(P>0.05).(3)In the OGTT test,the plasma glucose in control group was less than A,B and C groups at postprandial zero minutes,half-hour and two hours(P<0.05),C peptide in the control group was greater than the A,B and C groups at postprandial half-hour(P<0.05).(4)?C30/?G30,?C2h/?G2h,Homa2-%?and Homa2-%S in the control group were greater than A,B and C groups.Homa2-IR is less than A,B,C groups.Compared with C group,Homa2-%?was greater in the B group,Homa2-IR and Homa2-%S was smaller in the A and B groups(P<0.05).(5)The correlation analysis showed that GFR negatively correlated with age,disease duration,SBP,Scr,mALB,Homa2-IR and Homa2-%S,positively correlated with FPG,30minPG,2hPG,Homa2-%?.(6)GFR independently associated with the Age,Scr,mALB,30minPG,Homa2-IR and Homa2-%S(R=0.662,R2=0.438)of type 2 diabetes.Contribution of GFR:Age>Homa2-IR>Homa2-%S>mALB>Scr>30minPG.Conclusion:(1)There was single nucleotide polymorphism in betacellulin exon 1TGC19GGC gene.(2)Found no correlation between betacellulin(BTC)exon 1TGC19GGC gene polymorphism and type 2 diabetes,also found no correlation with diabetic nephropathy.(3)TT genotype perhaps may play a protective role in the basis insulin secretion of type 2 diabetes in Northern Anhui.
