1288 Cases Of Clinical Treatment Of Budd-Chiari Syndrome Were Retrospectively Analyzed

Objective: Summarize and discuss the effective diagnosis,treatments,and the factors that infect the prognosis and recurrence of Budd-Chiari syndrome.Methods: A retrospective analysis of 1288 cases of Budd-Chiari syndrome which were treated by The first affiliated Hospital of Bengbu Medical College from January2004 to October 2015,including diagnosis,treatment and follow-up results,with ?2test and signed-rank test as its statistical methods.Results: 1760 treatments were performed in this group with endovascular treatment for 1161 cases and surgical treatment for 74 cases while no treatment for53 cases.(1)There were 808 cases of Inferior vena cava obstruction.After endovascular therapy,787 cases of obstruction were successfully released and the success rate reached 97.40%.There were also 283 cases of hepatic veins obstruction and 70 cases of mixed.With endovascular surgery,264 and 64 cases of each type got successfully solved and the hit rates reach 93.28% and 91.43% for each.(2)Radical surgery were performed in 26 cases while bypass surgery were carried out in 48 cases and Transjugular intrahepatic portal-shunt(TIPS)for 11 cases.3 cases of severe esophageal varices and hypersplenism underwent splenectomy.The effective rate got96.81% in endovascular group and 97.30% in traditional group,showing no significant otherness(P> 0.05).1103 cases were followed up after surgery with 1029 cases for endovascular group and 74 cases for surgical group.The follow-up rate was 85.64%.The follow-up period raged from 1 to 104 months with an average for 40.3 months.During the follow-up,224 cases got recurrence,157 cases relapse within 2 years.The overall recurrence rate reached 20.31% and the 2-year recurrence rate showed14.23%.244 patients underwent repeated endovascular surgery,the proportion was23.71%.Conclusion: The endovascular surgery showed great advantage over traditionalsurgery in the treatment of Budd-Chiari syndrome and gradually became the first choice which used to be the later.The complex type of BCS should be treated individualizedly.For those patients who suffer from recurrence or re-obstruction,another endovascular treatment could brings an approving result.The elastic recoil of the vascular wall and the inflammation of injured vein wall are the main aspects which may related to the relapse of BCS.Meanwhile,the severity of liver damage shows great infect on the prognosis of BCS.
