Clinical Analysis Of Jaw Cyst Associated With Impacted Supernumerary Tooth

Objectives:To explore the clinical characteristics of jaw cyst associated with supernumerary teeth,and evaluate the therapeutic effects so as to provide reference basis to clinic.Method:26 cases of dentigerous cyst associated with supernumerary teeth were screened from the patients with dentigerous cyst and supernumerary teeth who were diagnosed at Department of Oral and Maxillofacial Surgery in Yanbian hospital from January 2005 to December 2014.Their age distribution,main symptoms,positions and sizes of lesions,therapeutic methods and effects,and follow-up results were analyzed.Results:1.The dentigerous cyst associated with supernumerary teeth was 26 and accounted for 19.7%of the dentigerous cyst(132 cases),for 49.1%of maxillary dentigerous cyst(53 cases)and for 8.8%of supernumerary teeth(294 cases).2.The main symptoms of patients were characterized by swelling(14 cases,53.8%);accidentally found or by conventional X-ray(9 cases,34.6%);painful with swollen cheek(3 cases,11.5%).The course of disease varies from 1 day to 5 years.3.All cases of dentigerous cysts caused by supernumerary teeth had all occurred in the anterior region of maxilla.Among them,6 cases expanded to the molar area.3 cases involved to the maxillary sinus,4 cases involved to the nasal floor,2 cases lead to facial dysmorohism,2 cases caused to malocclusion,8 cases induced pulp necrosis.4.The maximum cyst diameter(MCD)was varies from 2 to 10 cm,the mean diameter was 3.8 cm.The MCD was less than or equal to 3 cm in 12 cases(12 cases,oval);MCD was more than 3 cm in 14 cases(10 cases,lobulated;4 cases,oval).All cases of dentigerous cyst associated with supernumerary teeth,the one and two supernumerary teeth in the cyst was 24(93.3%)and 2 cases(6.7%).5.We performed The simple cyst curettage operation was 18 cases,cyst curettage+artificial bone implantation was 8 cases.The 19 cases was followed up(26 cases in total)and no recurrence case was found.6.When MCD was less than 3 cm,the healing grade was excellent treated by simple cyst curettage or curettage+artificial bone implantation in the 12 cases.When MCD was more than 3 cm,the healing grade was different treated by simple cyst curettage(8 cases),2 cases was excellent,5 cases was good and 1 case was poor.Treated by simple cyst curettage+artificial bone implantation in the 6 cases,4 cases was excellent,1 case was good and 1 case was poor.Conclusions:1.Supernumerary teeth should be removed as soon as possible.2.The radiographic examination had great significance in evaluation of diagnosis and treatment effectiveness in dentigerous cyst associated with supernumerary teeth.3.Artificial bone implantation was suitable for treatment on more than 3 cm in MCD jaw cyst,which had a better therapeutic effect in bone defect healing.
