| Purpose and significance:This study through the Ye Tianshi "clinical guidelines basis"dysphagia case found in Ye Tianshi is good at using ginger juice and bamboo juice treat dysphagia.Therefore,we use Xinhua Tongli Yin of the ginger juice and bamboo juice through drink dysphagia in patients with Esophagus-Cardia Cancer Dysphagia And Functional Delayed Gastric Emptyimg,the patients before and after medication was observed in the clinical efficacy,determination of gastrointestinal hormone changes,and by establishing a gastrointestinal disorder experimental animal model of gastric emptying retention rate of the determination of each group,the small intestine propulsion ratio,mice gastrointestinal hormone and so on,to prove the validity of Xinhua Tongli Yin,and discussed the mechanism,for the clinical application of Xinhua Tongli Yin treatment of esophagus cardiac carcinoma postoperative gastric kinetic energy barrier to provide experimental basis.Research Methods:Clinical studies was used.Methods established inclusion criteria,exclusion criteria Collected 13 cases esophagus cardia cancer patients with dysphagia,through oral Xinhua Tongli Yin were observed of dysphagia,nausea,vomiting,saliva,sputum hiccups belching,loss of appetite symptoms and the change of CEA,CA199 and SCC before and after treatment.Collection of 30 cases of esophageal cardia cancer patients with postoperative gastric emptying,were randomly divided into treatment group and control group,treatment group with oral Xinhua Tongli Yin treatment,the control group treated with Mosapride,through the improvement of clinical symptom scores of traditional medicine and KPS score,measured mice gastrointestinal hormone before and after treatment,observation of CEA,CA199 tumor index and untoward effect such as the abdominal pain and diarrhea,the liver and kidney function.Through the establishment of gastric dynamic obstacle model in mice,observed before and after treatment groups of gastric residual rate,small intestine propulsion rate and serum gastric dynamic element,element changes.Results:In the treatment of esophageal cardia cancer dysphagia,the total effective rate of Xinhua Tongli Yin was 76.9%,CEA,CA199,SCC by statistical test before and after the treatment has no obvious difference(P>0.05),but the tumor index is lower than before treatment after treatment.In the treatment of esophagus and cardia cancer postoperative gastric emptying,the treatment group clinical healing in 2 cases,3 cases had marked effect,9 cases of effective and ineffective in 1 case,the control group clinical recovery in 1 case,1 case had marked effect,10 cases were effective,ineffective in 3 patients,two groups of TCM symptoms curative effect by the statistical test has no obvious difference(P>0.05),but the total effective rate of treatment group is higher than the control group,treatment group effective rate was 93.33%,effective rate was 80%in the control group,especially in improving abdominal distension,early enough,poor appetite,nausea,anti acid heartburn symptoms,treatment group period is higher than the control group.Serum before and after the treatment group and control group in the treatment of gastric movement and gastric secrete element have examined the statistical significant difference(P<0.01),but in the treatment group and control group in the two groups after treatment of gastric dynamic element and the laboratory by the statistical test has no obvious difference((P>0.05).The tumor marker of CEA and CA199,KPS,untoward effect by the statistical tests no statistical difference(P>0.05),but the trend of the treatment group are better than the control group.In the animal experiments,each group before and after treatment of gastric residual rate,ratio,and small intestine propulsion gastric dynamic element,serum gastric secrete element by the statistical tests are obvious difference(P<0.01),the traditional Chinese medicine with Xinhua Tongli Yin through the low dose group is the best Chinese medicine.Research conclusion:The treatment of esophagus cardia cancer dysphagia and functional delayed gastric emptyimg by Xinhua Tongyin Yin has a good curative effect,can improve the clinical symptoms and patient quality of life,a kind of treatment of esophagus cardia cancer patients and postoperative gastric emptying disorder,and serum gastric dynamic element,serum gastric secrete element can be used as a curative effect evaluation index,worthy of clinical application and further study. |
