| Objective:This research is divided into clinical research and experimental research in clinical research on the two part,the First Affiliated Hospital of Guangzhou University of Chinese Medicine from 2009 January to 2011 December two Department of internal medicine,3 years for a clinical diagnosis of liver cirrhosis of 102 cases with sex,age,etiology,symptoms,clinical symptoms,syndromes and treatment law carries on the analysis and the summary,discuss"spleen","blood" in the diagnosis and treatment of liver cirrhosis in the process of the position and meaning;experimental study of Jianpi Huoxue Decoction Combined with MSCs through observation of transplantation on cirrhotic rats serum ALT,AST,ALB,TB,SDF-1 in liver tissue of hydroxyl proline,and the changes of CXCR4 content,as well as groups of liver tissue by HE staining,Masson dyeing,with green fluorescent protein markers of MSCs distribution in the liver,of Jianpi Huoxue Decoction Combined with MSCs transplantation in the treatment of cirrhotic rats with MSCs in the liver of colonization,the influence of liver function and liver cirrhosis degree mechanism,for clinical provides a powerful experimental basis.Method:1.Clinical retrospective study:all the cases from the First Affiliated Hospital of Guangzhou University of Chinese Medicine,two Department of internal medicine.The Haitai Jiahe system and hospitalized cases system examination of relevant cases,will meet the diagnostic criteria were included in this study were 102 cases.2.Animal experiments:49 SPF SD female rats were divided into control group(normal)(7 rats,group A)and model group(42 rats)in two groups of rats in the model group,with 40%CC14(CC14:olive oil=4:6)subcutaneous injection of 12weeks made of cirrhotic rats model,model group is again divided into 4 groups(group B-model group,C group-MSCs group,D group-Jianpi Huoxue Decoction group,E group-strengthening spleen and Activating Blood Prescription +MSCs),group C,group E rats via tail vein injection of MSCs,D group,E group of rats with spleen invigorating and blood circulation promoting prescriptions traditional Chinese medicine orally.Results:1.Clinical retrospective study:by retrospective study,we found in cirrhotic patients:age of onset is between the ages of 50-59peak,60-69 years time,description and the incidence of age related cirrhosis.A higher proportion of women in male patients with liver cirrhosis.Liver cirrhosis is one of main causes of chronic hepatitis B virus infection(59.8%),patients with alcoholic cirrhosis secondary(15.2%).Cirrhosis of liver disease mainly in the liver and spleen,heart,lung and kidney,but related,symptoms mainly abdominal distension,tired,poor appetite,eat less,oliguria,edema and other symptoms,to the accumulation of damp-heat syndrome,blood stasis syndrome of drug use,Rhizoma Atractylodis Macrocephalae,tuckahoe,liquorice,Polyporus,Alisma,wormwood,madder root,Danshen,Pericarpium Arecae,dragon,Zeeland,such as Radix Pseudostellariae,description of histologic cirrhosis is one of main factors of damp-heat,blood stasis,wet,use is made of Qi and strengthening spleen,activating blood circulation to dissipate blood stasis,clearing heat and removing dampness,eliminating dampness through diuresis medicine.Therefore,we in the treatment of liver cirrhosis should pay attention to the process of" wet""blood stasis" two aspects,medication to pay attention to the use of" spleen" and" blood" to herbal prescription.2.The experimental results show that:Green fluorescent protein marker MSCs in group C,group E in liver tissues of rats with colonization.The rats in group E in the serum content of ALT compared with A group had no statistical significance(P>0.05),and B decreased significantly(P>0.01),compared with C group decreased(P<0.05);Serum AST content was not statistically significant(P>0.05);E rats serum ALB content and A group still reduced(P<0.05),compared with B group increased(P<0.05),compared with C group had no statistical significance(P>0.05);E rats serum TB content compared with other groups was not statistically significant(P>0.05).The rats in group E in liver tissue hydroxyproline content compared with that in A group increased significantly(P<0.01),compared with B group decreased(P<0.05)compared with C group,although the content of decline,but no statistical significance(P>0.05).Various immunohistochemical SDF-1,the positive expression of CXCR4cell count was found when comparing,the positive expression rate of E group was higher than that in group A,group B,group C(P<0.01).Pathology showed:the rats in group E than in group A of the hepatic lobule structure damage,compared with group B hepatic lobule structure damage is reduced somewhat,fiber cable loose narrows,hepatocyte edema improved,degeneration of improved significantly,and the collagen fiber was less,interval,no false lobule formation,hepatic lobule structure is relatively a relatively complete,compared with the C group showed no significant difference.Conclusion:The study of 106 cases of cirrhosis cases carried out a systematic clinical retrospective study,the results are as follows:1.The peak age of onset for cirrhotic patients aged between 50-59 years,60-69 years old times.Description and the incidence of age related cirrhosis.2.Male patients with cirrhosis than in female patients.3.Liver cirrhosis is one of main causes of chronic hepatitis B virus infection(59.8%),alcoholic liver cirrhosis patients accounted for 15.2%,is second,that of Chinese patients with liver cirrhosis is mainly caused by hepatitis B virus infection in patients with alcoholic cirrhosis,but the number is on the rise.4.Cirrhosis of liver disease mainly in the liver and spleen,heart,lung and kidney,but related,symptoms mainly abdominal distension,tired,poor appetite,eat less,oliguria,edema and other symptoms.5.Syndrome of damp-heat accumulation to give priority to,blood stasis syndrome of drug use,Rhizoma Atractylodis Macrocephalae,tuckahoe,liquorice,Polyporus,Alisma,wormwood,madder root,Danshen,Pericarpium Arecae,dragon,Zeeland,such as Radix Pseudostellariae,description of histologic cirrhosis is one of main factors of damp-heat,blood stasis,water wet.Use is made of Qi and spleen,promoting blood circulation and removing blood stasis,clearing heat and removing dampness,eliminating dampness through diuresis medicine.Therefore,we in the treatment of liver cirrhosis in the process must pay attention to "wet"" blood stasis" two aspects,medication to pay attention to the use of" spleen" and" blood" to herbal prescription.6.Effect of Jianpi Huoxue Decoction Combined with MSCs transplantation group of liver histopathology revealed hepatic lobule structure damage is reduced somewhat,fiber cable loose narrows;hepatocyte edema improved significantly improved,degeneration,inflammatory cell infiltration reduction.The collagen fibers were less,spaced,central venous see small amounts of collagen tissue to hepatic sinus area extending,but has not formed the winding,no false lobule formation,hepatic lobule structure is relatively complete,combination therapy with improved stiffness,reduced collagen deposition.7.Effect of Jianpi Huoxue Decoction Combined with MSCs in treating cirrhotic rats with cirrhosis of liver can reduce the ALT content,and increase the content of ALB,description of the combined treatment can improve the liver function in cirrhotic rats.8.Effect of Jianpi Huoxue Decoction Combined with MSCs transplantation group in liver tissue of rats liver tissue by immunohistochemistry SDF-1,the positive expression of CXCR4 cell count was significantly higher than that in the model group and MSC transplantation group,that combination therapy may be increased through the liver tissue SDF-1 content,improve the MSCs"homing";activation of SDF-1/CXCR4mediated signal transduction pathway,thereby improvement in cirrhotic rat liver function and reduce the degree of hepatic cirrhosis. |
