The Case Analysisi Of Serology Tumor Marker(Ca153、Ca125、Ca199、CEA、AFP)in 936 Hepatic Lesions

Objective:To discuss the effect for antidiastole of serology tumor marker(Ca1531、Ca125、Ca199、CEA、AFP)for several malignant hepatic lesions.By means of analysis the distribution of several serology tumor marker in all kinds of benignant and malignant hepatic lesions to find out which of these tumor marker has effect on Differential diagnosis meaning to the malignant hepatic lesions.And ascertain it’s diagnostic boundary value.Method:To review the case data of The 1st afflicted hospital of Guangxi medical university which were definite diagnosed as HCC(Hepatocellular Carcinoma)、Cholangiocarcinoma、hepatic metastasis、cavernous hemangioma of the liver、Hepatic Adenomas、cyst of liver、FNH(focal nodular hyperplasia).There are altogether 936 cases:Cholangiocarcinoma 129 cases,HCC 579 cases,hepatic metastasis 67 cases,cavernous hemangioma of the liver 86 cases,Hepatic Adenomas 9 cases,cyst of liver 86 cases,FNH 3cases.All of the caes above were made definite diagnose.We analyze the data of the case above.Using the malignant cases:HCC,Cholangiocarcinoma,hepatic metastasis,as experimental group,and benign cases:cavernous hemangioma of the liver、Hepatic Adenomas、cyst of liver、FNH as matched group.Result:After analyze come to a conclusion that the diagnostic boundary value of Ca125 towards Cholangiocarcinoma is 13.97,the sensitivity is 85%specificity is 82.1%.Ca199 the diagnostic boundary value is 17.52 the sensitivity is 77.5%specificity is 82.1%.when it comes to the case of HCC,the best tumor marker for differential diagnosis is AFP,it’s diagnostic boundary is 4.26 the sensitivity up to 86%,specificity is 89.3%in the study.Besides,in these HCC patients whose HBsag is positive,the diagnostic boundary is 4.26,sensitivity is 86%,specificity is 89.9%,the patient whose HBsag is negative,the diagnostic boundary is 4.28,sensitivity is 67.9%,specificity is 89.3%.In the case of hepatic metastasis the diagnostic boundary of CEA is 2.23,the sensitivity is 61.1%,specificity is 60.7%.Ca199 the diagnostic boundary is 13.78,sensitivity is 61.1%,specificity is 67.9%.Conclusion:As preliminary screening,the diagnostic boundary value of Ca125 towards Cholangiocarcinoma is 13.97,Ca199 17.52;HCC the best tumor marker for differential diagnosis is AFP,it’s diagnostic boundary is 4.26;In the case of hepatic metastasis we did not find any useful tumor marker for Antidiastole.
