Objective:1.To conduct systematic review of the research on the root canal configuration of all the mandibular first premolars,gain a better understanding of the general situation of root canal incidence,combine the incidence rate of root canal configuration among different research and analyze the influence of different race on root canal variation through evidence-based medicine method.2.The cone beam CT is applied to investigate the root canal configuration of the mandibular first premolars in Luzhou regions,the symmetry of root canal and the impact of gender on root canal variation so as to provide guidance for the dental clinical services and fundamental statistics for the improved systematic review in this region.Methods:This research includes two parts.Part ?:1.Formulate the search strategy,search the Medline(1950-2014),EMBASE(1950-2014)and other foreign language databases,the CBM,CNKI and other Chinese database as well as the relevant conference papers and the unpublished grey literature so as to obtain the research-related literature.2.In strict accordance with the inclusion and exclusion criteria,two researchers receiving professional training offer quality evaluation to the bias risk assessment standards of Australian Joanna Briggs Institute(JBI)instead of eliminating the literature according to the quality.3.Revman5.3 is adopted to offer Meta analysis of the root canal incidence rate under the random effects model and to conduct subgroup analysis in accordance with difference race.Part ?:The scanning information of 872 root canal configuration of all the mandibular first premolars of 436 patients coming to our hospital and undergoing CBCT from June 2012 to December 2013 is collected to record gender,analyze the root canal number,symmetry and the difference in the root canal configuration proportion among people of different genders.Results:Part?:1.There are 23 literatures from 8 countries and 7,475 cases of samples in this research.2.The pooled incidence rate is 73%(95%CI68%,78%)in type ? root canal,3.85%(95%CI2.91%,6.54%)in type ? root canal,2.91%(95%CI 1.96%,3.85%)in type ? root canal and 2.91%(95%CI 1.96%,5.66%)in type ? root canal,12.3%(95%CI9.09%,15.25%)in type V root canal and 0.99%(95%CI 0,1.96%)in type ? root canal,0.99%(95%CI 0,1.96%)in type ? root canal and 0.99%(95%CI 0,1.96%)in type ? root canal.There is no statistical difference of subgroup analysis in the above-mentioned groups(p>0.1).The pooled incidence rate is 5.66%(95%CI0.99%,32.0%)in type ? root canal and 1.96%(95%CI 0.99%,2.91%)in other groups with statistical difference in the analysis of subgroups(p<0.1).Part ?:1.In line with the Vertucci classification method,the incidence rate of root canal of mandibular first premolars is 85.67%in type ?,3.10%in type ?,0.03%in type IV,8.00%in type V and 2.87%in C-type.2.There are 367 cases of patients with symmetric type ? root canal of mandibular first premolars,10 cases with symmetric type ? root canal,1 case with symmetric type IV root canal,25 cases with symmetric type V root canal and 7 cases with symmetric C-type root canal.There is significant difference in the symmetry of 5 types of root canal of mandibular first premolars(P<0.001).3.There is no statistical difference in the constituent ratio of the root canal configuration of all the mandibular first premolars among people with different genders(P>0.05).Conclusions:1.The existing research results show that there is a high degree in the root canal variation of mandibular first premolars;however,the significant difference of the incidence rate of root canal configuration among different races is still not defined.2.It is essential to collect as comprehensive evidence as possible for the research.3.Observational research system evaluation should be further improved.4.Considering the complex root canal system of the mandibular first premolars in Luzhou regions,the systematic data of the root canal of mandibular first premolars in Luzhou undergoing the CBCT analysis provide theoretical basis for root canal therapy.
