Part1.Multivariate analysis of advanced esophageal cancer based on 19 prognostic markersObjective:To investigate the relationship between clinicopathologic characteristics,preoperative inflammatory factors as well as serum tumor markers and prognosis of advanced esophageal cancer.Methods:A total of 304 patients with advanced esophageal cancer in Nanjing Drum Hospital from May 2008 to June 2013 were retrospectively analyzed in this trial.The data concerned clinicopathologic characteristics,inflammatory factors and serum tumor markers.These patients were followed up by telephone and outpatient clinic.The results were analyzed by SPSS 17.0 statistic software.Survival curves were constructed in terms of the method of Kaplan-Meier.Univariate and multivariate analysis was performed by Log-rank test and Cox regression analysis,respectively.Results:R0 excision,N stage,M stage,maximum tumour diameter,nerval invasion,vascular invasion,preoperative neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR),albumin,carcinoembryonic antigen(CEA)and carbohydrate antigen(CA)125 were related to overall survival in patients with advanced esophageal cancer in univariate analysis.Multivariate analysis revealed that non-RO excision,N3 stage,M1 stage,maximum tumour diameter,vascular invasion,NLR were independent progonostic factors for overall survival after esophageal cancer resection.Conclusion:R0 excision,N stage,M stage,maximum tumour diameter,nerval invasion,vascular invasion,NLR,PLR,albumin,CEA and CA125 were valuable to evaluate prognosis of esophageal carcinoma.Part 2.Expression of vascular endothelial growth factor and its correlation with clinicopathological characteristics in esophageal squamous cell cancerObjective:To investigate the expression of vascular endothelial growth factor(VEGF)and its relationship with clinicopathological characteristics in esophageal squamous cell cancer.Methods:Tissue samples of primary tumors from 265 esophageal squamous cell cancer patients undergoing curative surgical resections were immunohistochemically detected for VEGF,epidermal growth factor receptor(EGFR)and cyclooxygenase-2(COX-2),etc.The correlation between the expression of VEGF and clinicopathological characteristics were analysed by chi-square test.Results:The positive expression rate and elevated expression rate of VEGF in esophageal squamous cell cancer were 87.9%and 58.1%,respectively.The elevated expression of VEGF was statistically correlated with TNM stage,lymphatic metastasis,serosal invasion,vascular invasion,EGFR expression and COX-2 expression.Conclusions:The VEGF expression level in esophageal squamous cell cancer might be associated with tumor stage as well as invasion and be served as an important reference index predicting its biological behavior. |
