A Study On The Public Health Service Of Public Hospitals At Level 2 In S City Of Shanxi Province

ObjectiveThis study is based on the determination of the functional orientation of public hospitals in the public health service system and the utilization of public health service resource allocation and the residents' use of public hospital public health service projects in the survey of 1 3 secondary public hospitals in S city of Shanxi Province.The current problems are identified and corresponding countermeasures and recommendations are proposed to improve the region,provide a reference for improving the capacity of regional public hospi tals and health services.MethodsThe research method of this topic is mainly based on literature research method,empirical analysis method,interview method,questionnaire surveys and statistical analysis.The case and empirical analysis are based on t he public hospitals of Level 2 in S City as the research object.Analysis and literature studies include access to relevant government policy documents,report books,and related journals.Surveys and researches are mainly conducte d through questionnaire surveys and interviews with subjects.Questionnaires were used to investigate the residents' use of public health services in public hospitals.Data acquisition uses data collection and analysis methods applied to data tools for input analysis.Through the application of various research methods,the problems existing in the public health services of public hospitals in Level 2 in S City were analyzed.ResultsFunctional orientation:The public health functions of secondary public hospitals in S city play a very important role in the city's public health system.While vigorously developing the capacity of public health services at basic medical institutions,we must pay attention to the development of public health in public hospitals.The allocation of resources:1.The management system of public hospitals in Level 2 in S City is incomplete,and the resources for public health services are not balanced.There are 9 hospitals in the city set up public health departments,accounting for 69.2%.The range of public health service projects is relatively comprehensive,but some special public health projects are not satisfactory: 53.8% of the hospitals have carried out the project of managing the mental health of severe mental patients,and 53.8% of the hospitals have carried out support and support for agricultural projects.46.2% of the hospitals carried out two cancer screening projects.2.There is a large gap in public health professionals.There are 2,790 medical and health workers in public hospitals of Level 2 in the city,and 62 public health professionals,accounting for 2%,of whom only 4 are public health related professionals,compared with forecasted demand,far reaching the demand of local residents for public health services.3.The government does not invest enough in public health in public hospitals.In some areas,public MCH hospitals,Chinese medicine hospitals,especially Psychiatric hospitals,and infectious disease hospitals have difficulties in business development.In the government's fiscal compensation for public hospitals,the scope of compensation is narrow and the compensation method is chaotic.It cannot solve the problem of uneven distribution of public health service resources.Utilization of resources:4.The survey results showed that the 491 respondents had a relatively high utilization rate of public health service projects in public hospitals,and several of them with low utilization rates may be related to the sinking of basic public health service resources to grassroots heal th institutions.Residents have a higher degree of satisfaction with service attitude and service technology of public health service personnel in public hospitals.However,the service technology requirements are higher.The factors influencing the degree of satisfaction with public health service attitudes include the residents' educational level,residents' area,and residents' habitual residence.ConclusionsFor public hospitals,improve their comprehensive health service levels is a relatively complex project,the need to adopt the method of step by step,constantly improve the health service ideas based on the premise of health services,and provide the corresponding institutional guarantee,policy support,service mechanism,supervision mechanism,aim ing at the weak spot,enhance the comprehensive ability to public hospitals and health services.1.Reform the management system of public health services,increase the establishment standards of public health units,and proceed from the three links of infrastructure standards,staffing standards and service quality standards.Improve the capacity of public health services in public hospitals.2.To cultivate professional experts in public health services,focusing on improving the entry threshold for emplo yees,refining the division of services,and implementing the preparation and preparation system.The practitioners should focus on the entry criteria for practicing physicians and registered nurses as physicians and nurses.Establish a team of experts to establish a vertical referral model within the expert team and effectively use experts' resources at all levels.3.Increase the government's investment in public health,improve the government compensation mechanism,reform the subsidy method,adjust the direction of the use of funds,and implement the costs of public hospitals in public health basic construction and personnel training.4.Establish a sustainable operating mechanism,starting from the four links,effectively transforming the income sources of public hospitals,controlling unreasonable cost growth in public health services,setting up public health service funds,and promoting the participation of social forces in public health services.At the same time improve the supervision system of pub lic health services,set up a special supervision agencies,clear the principle of punishment,strengthen the implementation of responsibilities,and strengthen the performance assessment of medical personnel.
