Objective: To study the characteristics of bone marrow microenvironment with radiomics based on IVIM mapping in different hyperplastic marrow.Compared with the results of histological cellularity and MVD,the clinical value of texture analysis of radiomics to evaluate hyperplastic marrow was analyzed.Material and Methods: From the department of Hematology of the Second Hospital of Shanxi Medical University,28 newly diagnosed patients with acute leukemia(AL)and 15 newly diagnosed patients with proliferative anemia were collected.MRI scan of the lumbar spine was performed before chemotherapy.The scan sequences included a sagittal SE T1 WI and a sagittal IVIM based multi-b value DWI(b=0,10,25,50,100,200,400,600,800,1000,1200s/mm2,gradually increasing a total of 11 b values).At the end of the scan,the D,D*,and f values of the bone marrow of the lumbar 2,3,and 4 vertebrae were measured.D mapping,D* mapping and f mapping were reformated by the workstation and imported into Ma Zda software for texture analysis.The texture analysis was used to study the characteristics of bone marrow microenvironment.The smears were stained with CD34,and the microvessel density of the bone marrow were counted under a microscope at a magnification of 100 and 200.The sections were stained with HE,and the bone marrow cellularity were counted under a microscope at a magnification of 200 and 400.Results:1.Bone marrow cellularity of AL group was significantly higher than that of anemia group(t=3.330,P=0.004).There was no significant difference in the D value between the AL group and the hyperplastic anemia group(t=0.504,P=0.617).However,Sum Averg,Horzl Glev Non U,135 dr GLev Non U,Z RLNon Uni of D mapping in AL group was higher than anemia group,and P value was 0.011,0.027,0.041,0.038,respectively.Using receiver operator characteristic(ROC)analysis,the area under the ROC curve(AUC)of Sum Averg,Z RLNon Uni of D mapping was 0.717,0.731,respectively,in evaluating the nature of bone marrow hyperplasia.2.MVD of AL group was significantly higher than that of anemia group(t=6.695,P<0.001).The f value of the AL group was higher than that of the proliferative anemia group(t=3.546,P=0.001).Correlat,Z RLNon Uni,Z GLev Non U of f mapping in AL group was higher than the anemia group,P value was 0.003,0.023,0.023,respectively.Using ROC analysis,the AUC of Correlat,Z RLNon Uni,Z GLev Non U of the f mapping was 0.743,0.710,0.700,respectively,in evaluating the nature of bone marrow hyperplasia.The AUC of f was 0.752,and greater than the texture parameter of f mapping.3.There was no significant difference in the D* value between the AL group and the hyperplastic anemia group(t=-1.787,P=0.081).Horzl GLev Non U,Vertl Glev Non U,45 dgr GLev Non U,135 dr GLev Non U,Z RLNon Uni,Z GLev Non U of D* mapping in AL group was higher than the anemia group,P value was 0.006,0.010,0.017,0.007,0.042,0.034,respectively.Using ROC analysis,the AUC of Horzl GLev Non U,Vertl Glev Non U,45 dgr GLev Non U,135 dr GLev Non U,Z RLNon Uni,Z GLev Non U of D* mapping was 0.738,0.724,0.714,0.752,0.690,0.721,respectively,in evaluating the nature of bone marrow hyperplasia.Conclusion: The IVIM-DWI technique can non-invasively assess bone marrow hyperplasia,in which parameter f has a certain value for the evaluation of the nature of bone marrow hyperplasia.The texture analysis of D mapping,D* mapping,and f mapping can analyze the heterogeneity of the bone marrow microenvironment,and can reflect the biological characteristics of the bone marrow microenvironment.
