Objective:By measuring serum fgf21 and adiponectin levels in patients with diabetic kidney disease,type 2 diabetes mellitus,and healthy people,the aim was to investigate the correlation between serum fgf21 and adiponectin levels and diabetic kidney disease.Methods:A total of 47 patients with type 2 diabetes mellitus diagnosed in the endocrinology department of the Second Hospital of Shanxi Medical University from October 2016 to October 2017 were collected.According to the urinary albumin excretion rate(AER),25 patients were classified as diabetic kidney disease(DKD),22 patients were classified as type 2 diabetes mellitus(T2DM).In the same period of time,25 healthy people examined at the physical examination center of the Second Hospital of Shanxi Medical University were selected as the normal control group(NC).Collect general data such as gender,age,height,weight,systolic blood pressure(SBP),diastolic blood pressure(DBP),duration of disease,calculate body mass index(BMI),and measure fasting blood glucose(FPG)and fasting insulin(FIns),Hb A1 c,urinary albumin excretion rate,serum creatinine(Scr),triglyceride(TG),total cholesterol(TC),high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C),and other laboratory tests and serum fgf21,adiponectin levels.Results:1.There was no significant difference between the genders in each group(P>0.05).The age,BMI,TG,FPG,and Hb A1 c in type 2 diabetes mellitus group were significantly higher than those in the normal control group(P<0.05).The HDL in the type 2 diabetes group was lower than that in the normal control group.The age,BMI,TG,FPG,Hb A1 c,and systolic blood pressure in diabetic kidney disease group were higher than those in normal control group respectively(P<0.05).The HDL in diabetic kidney disease group was lower than that in normal control group(P<0.05).The age,FPG,Hb A1 c,and systolic blood pressure of diabetic kidney disease group were higher than those of the type 2diabetic group respectively,and the differences were statistically significant(P<0.05).2.The level of FGF21 in type 2 diabetes group was higher than that in the normal control group(P<0.05).The level of FGF21 in diabetic kidney disease group was higher than that in the normal control group,and the difference was statistically significant(P<0.05).Adiponectin was lower in type 2 diabetes group than in the normal control group(P<0.05);Adiponectin was higher in diabetic kidney disease group than in type 2diabetes mellitus group,and the difference was statistically significant(P<0.05).3.Adiponectin was positively correlated with the course of disease and HDL;FGF21 was negatively correlated with HDL;FGF21 was positively correlated with BMI,AER,and TG.4.AER was used as the dependent variable and multiple linear regression analysis was performed with disease course,Hb A1 c,HOMA-IR,adiponectin,and FGF21 as independent variables.No independent risk factors for the occurrence and severity of DKD were found.Conclusion:Adiponectin is a protective factor in the body and may be a serological marker for the development of T2 DM and diabetic kidney disease.FGF21 may be involved in the progression of DKD,and elevated levels may indicate the occurrence and development of DKD.
