This paper is consisted of two parts: the theoretical exploration and animal experiment of treating insulin resistance in type 2 diabetes mellitus with method of assisting the spleen to disperse the essence.Part one: theoretical explorationThis part introduces the method of assisting the spleen to disperse the essence for the treatment of insulin resistance in type 2 diabetes mellitus based on the Traditional Chinese Medicine theory and my supervisor's perspective and clinical experience.Part two: animal experimentObjectives: Inspired by the theory of the failure of the spleen to disperse the essence,which was proposed by Professor and contributed to the pathogenesis of insulin resistance and diabetic mellitus,we established the treating principle of assisting the spleen to disperse the essence and chose Banxia Xiexin Decoction(Pinelliae Decoction for Purging Stomach-fire)as the basic intervention.The effect of the principle of assisting the spleen to disperse the essence was observed on insulin resistance in rat models of type 2 diabetes mellitus and related TNF-?,IL-6,FFA and NO.The efficacy of this treating principle in relieving insulin resistance in rat models type 2 diabetes mellitus was observed and explored the related mechanism.This was to provide theoretical evidence and expanding the treatment for insulin resistance in diabetes mellitus from the perspectives of inflammation,lipid metabolism and oxidative stress.Method:SD rats were selected and randomized into the black control group and the diabetic model group.Rats in the diabetic model group were fed with high-fat and high-sugar diet for 2 weeks to induce insulin resistance.With only water-derivation for 12 hours,rats with insulin resistance were intraperitoneally injected STZ at the dose of 40mg/kg.Three days later,diabetic models were established in rats with FBG ranged from 16.7mmol/L to 30.0mmol/L.Then the rats models with type 2 diabetes mellitus were randomly divided into the model control group,the positive control group,the high dose of Banxia Xiexin Decoction group,the moderate dose of Banxia Xiexin Decoction group and the low dose of Banxia Xiexin Decoction group.Each group was given normal saline,metformin and Banxia Xiexin of high,moderate and low doses respectively for 2 weeks.FBG and relate parameters were recorded before and after the experiment.Enzyme-linked immunosorbent assay was used to detect the levels of serum insulin,TNF-?,IL-6,FFA,NO.Results: 1.The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to stabilize the rat weight and decrease the mortality rate in the rats.2.The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to decrease the FBG in diabetic rats and relieve the insulin resistance the diabetic rats.3.The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to decrease the levels of TNF-? and IL-6,especially TNF-? and therefore relieved the inflammation in diabetic rats.4.The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to decrease the level of serum FFA in diabetic rats,inhibit fat disengagement and relieve lipid toxicity.5.The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to increase the level of NO in diabetic rats.6.These effects of The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence were dose-effect dependent.These effects in the moderate dose group were greater than those in the high and low dose group.Conclusion: The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was able to improve the general conditions of diabetic rats,regulate the blood glucose levels of diabetic rats and significantly decrease the insulin resistance index.It was also effective in decreasing the levels of TNF-?,IL-6,FFA and NO.It was suggested that The Banxia Xiexin Decoction with the treatment principle of assisting the spleen to disperse the essence was possibly to influence inflammation reaction,lipid metabolism and oxidative stress by insulin related signaling pathway,resulting in relief of insulin resistance and diabetic pathogenesis.
