Objective To explore the clinical characteristics and treatment of pediatric intraspinal space-occupying lesions,and then to improve the clinical cognition and therapeutic options.Methods The clinical data of 27 pediatric space-occupying lesions cases in the General Hospital of Ningxia Medical University from January 2010 to February 2018 were analyzed retrospectively.Results The 27 patients including 14 female children(51.85%)and 13 male children(48.15%).The age group was divided into 0~4 years old age group 4 cases(14.81%),5~9year old age group 6 cases(22.22%),10~14 age group 17 cases(62.97%).The most common symptoms include the progressively declined muscle strength 18 cases(66.67%)and the pain17 cases(62.96%).Preoperative MRI was routinely performed in all patients.10 cases were intramedullary lesion occupying(37.04%),10 cases were extramedullary space-occupying(37.04%),7 cases were epidural space-occupying(25.93%).The tumors focus of 19 cases were completely resected,15 cases were subtotally removed,one case partial excision.All of them achieved temporary postoperative relief.2 cases of puncture biopsy were performed for the invasion of the malignant tumor of the posterior mediastinum and the thoracic vertebra to the spinal canal,and the chemotherapy regimen was given.4 patients received postoperative radiotherapy and one patient received chemotherapy.Post-operative pathological:there were spinal tumors in 23 cases(85.19%),non-neoplastic spinal occupying in 4 cases(14.81%).The top 3 pathology types are: embryonic residual tumor in 6 cases,ependymoma in 5 cases,primitive neuroectodermal tumor in 3 cases.Conclusion A low incidence of pediatric intraspinal space-occupying lesions,the clinical manifestation was not typical.The most common clinical symptom was progressively declined muscular strength and pain,The most common type was embryonic residual tumor.The majority of non-tumor occupying positions were inflammatory granuloma.Surgical resection is the first selection to cure pediatric intraspinal space-occupying lesions.Malignant tumor with poor prognosis,a combination of surgical resection and chemoradiotherapy can effectively improve the survival time. |
