Objective:To analyze the change of serum IgA/C3 ratio in children with Henoch Schonlein purpura,and to assess the value of serum IgA/C3 ratio in predicting early renal injury.Methods:From March 2014 to November 2017,155 children with HSP in the department of pediatrics of first hospital of Shanxi Medical University were collected.HSP meet the standards of dignostic critreia with "Zhu Futang Practical Pediatrics diagnostic criteria" by Hu Yamei.All of the HSP children were divided into non allergic purpura nephritis group(group NHSPN,117 cases)and Henoch Schonlein purpura nephritis group(group HSPN,38 cases)by the abnormal urine test results.Collect the laboratory indexes(IgA,C3,ALB,GLB,TC,TG,Urea,CRE,Ig G,IgM,IgE,C4,ESR,PT,FIB,APTT,D-Dimer,WBC,PLT)at the time of children's admission to hospital,meanwhile to alculate the ratio of IgA/C3.Finally,using SPSS16.0 to analyze data,and according the date to analysis the clinical significance.Results:1.The serum IgA/C3 ratio was statistically different between the NHSPN group and the HSPN group,P<0.001.ROC analysis showed that higher sensitivity of the serum IgA/C3 ratio larger than 2.35 on the prediction of early renal injury in children with Henoch Schonlein purpura(AUROC: the ratio of serum IgA/C3,AUC=0.748;95%CI:0.672-0.824);2.Single factor analysis indicated that the serum levels of C4,ALB,TC,TG are significant different between the serum IgA/C3 ratio <2.35 group and the serum IgA/C3 ratio ? the 2.35 groups(P < 0.05).Correlation analysis showed that there was a correlation between C4 and the serum C3,and the ratio of serum IgA/C3(P < 0.05).The linear correlation regression analysis showed that IgA/C3=2.97-3.06 x C4.Conclusion:1.The serum IgA/C3 ratio in HSPN group is higher,and the sensitivity and specificity for the prediction of early renal injury in children with HSP are significant,so the serum IgA/C3 ratio larger than 2.35 may be used as the predictive indexes of early renal damage in HSP children in future.2.The correlation between the ratio of serum IgA/C3,C3 and C4 indicates multiple indexes may be more comprehensive in response to early disease activity in children with renal impairment.
