Research On The Relationship Between Endometrial Reception And Pregnancy Outcome Of Patients With URSA

Objective:To analyze the relationship between endometrial reception and pregnancy outcome of patients with unexplained recurrent miscarriage(URSA)by the transvaginal ultrasonic indicators,including endometrial thickness,type and blood flow signals.Methods:Monitor patients with RUSA through transvaginal ultrasound during December 2016 to August 2017.The endometrial thickness,type,uterine artery blood flow index(RI PI S/D)and subendometrial blood flow index(RI PI S/D)of them were record at the time of peri ovulation and implantation window.49 of them were pregnant and were chosen for the research.Divide them into two groups according to the gestational state of 20 weeks: abortion group(n=26)and gestation group(n=23)and compare their differences.Statistical analysis was carried out using the SPSS18.0 software,with measurement data treated by ttest,enumeration data treated with x2-test.AUC was used to judge monitoring value.Results:(1)The endometrial thickness in abortion group is thinner than gestation group(P > 0.05);(2)The rate of persistent gestation in type A and type B is higher than type C,but there is no statistically significant difference;(3)The uterine artery blood flow PI in gestation group is much smaller than the spontaneous abortion group(P < 0.05).The AUC in the period of peri ovulation and implanting window is 0.698 and 0.768;(4)Compare the differences of these parameters by distinguish endometrial type: the uterine artery PI of type A in the period of peri ovulation is higher in abortion group(P < 0.05),AUC= 0.756;the uterine artery PI of type B in the period of implantation window is higher in abortion group(P < 0.05),AUC= 0.734;At the time of peri ovulation period,the endometrial thickness of C type in abortion group is significantly higher than gestation group,AUC = 0.708;(5)Compare the differences of these parameters by distinguish the endometrial thickness: patients got pregnant whose endometrial thickness is less than 7 mm during the period of peri ovulation are type A;patients with type A whose endometrial thickness are between 7 to 10 mm and patients with type B whose endometrial thickness are between 7 to 14 mm have a higher rate of persistent gestation,but there is no statistically significant difference.Conclusions:At the time of peri ovulation period,the endometrial thickness of C type in abortion group is significantly higher than gestation group.At the time of implanting window period uterine artery PI in abortion group is higher than gestation group.These have predictive value for URSA pregnancy outcomes to a certain degree.
