Objective:Acute fatty liver of pregnancy(AFLP)is an uncommon but potentially fatal disease unique to pregnancy that typically occurs in the third trimester.The aim of this study was to explore the clinical features and risk factors of AFLP.Methods:We retrospectively analyzed 74 patients with AFLP,who received treatment from July 2006 to July 2017 in the Affiliated Hospital of Qingdao University and Qingdao Municipal Hospital,collecting clinical material such as clinical manifestations,laboratory examination,treatment,maternal and infant prognosis.And we randomly selected seventy-four pregnant women with normal hepatic function as the control group.Then we collected information of patients and analyzed the blood lipid level,weight gain,gestational hypertension,multiple pregnancy,and previous hepatitis B history.Firstly,the univariate factor analysis was used for related factors,then results that were significant difference were performed in multivariate analysis of unconditioned Logistic regression analysis.It was done with SPSS22.0 statistical software.Results:Most patients present with non-specifc symptoms such as nausea and vomiting,diarrhea,fatigue,itching and jaundice.Laboratory examination reveals leukocyte elevation,coagulation dysfunction,abnormal liver function,elevated blood creatinine,hypoglycemia and other abnormalitie.Mothers required admission to Intensive Care Unit(ICU)and transfusion of blood products,aggressive correctionof hypoglycemia,mechanical ventilation for Acute Respiratory Distress Syndrome(ARDS),dialysis,or plasma pheresis;Comparing the data of the AFLP group and the control group,the proportion of blood lipid level,weight gain,combined with gestational hypertension and multiple pregnancy were significantly different(P < 0.05).Multiple factors regression analysis showed that weight gain > 18 kg(OR = 4.442),combined with gestational hypertension(OR =4.899),merge multiple pregnancy(OR = 6.155)are in dependent risk factors for acute fatty liver disease during pregnancy.Conclusions:Multiple pregnancies,gestational hypertension,gestational weight gain > 18 kg are high risk factors for AFLP.Early attention to these risk factors may reduce and prevent the onset of AFLP.
