Acute Fatty liver of Pregnancy(AFLP)is a characteristic disease in third-trimester.The average gestational age at onset is28-40weeks and the overall incidence is1/10000-1/15000.The mortality-rate reaches high up to85%. Therefore familiaring the clinical characteristics and early diagnosis of AFLP and prompt comprehensive treatment appear particularly important.In recent years, the reports in domestic and foreign of AFLP and related research are in an increasing number and the maternal and fetal death rate had dropped significantly.But AFLP onset abruptly and has much complications,therefore the early diagnosis and treatment of AFLP are still the key points and difficulties in the clinical work.Objective:Discuss the gestational acute fatty liver characteristics of clinical cases and relatively analyzed.Compare these clinical cases of different times in order to explore the progress which was made in AFLP treatment in order to further strengthen the perinatal management provide clinical evidence.Methods:1.This article selected19clinical cases which are all definitely diagnosed for gestational acute fatty liver in the second and first hospital of Jilin University from2002-2011.The case features(including maternal age,clinical functions,liver and kidney function examinations,treatment,the Apgar score of fetus etc)are analysed by retrospective study.2.To divide the clinical cases into A group and B group.(A group including the clinical cases from2002-2006, B group including the clinical cases from2007-2011).Comparing group A and B(diagnosis,treatment)in order to explore the progress of AFLP treatment. Result:The maternal age is between21to39-year-old,(average is27.20士6.05) years old,80%of them is primipara.19patients appear obvious enteron symptom, accounting for82%;11patients appear lower limbedema, accounting for47.8%; All of the patients present with liver or(and) renal abnormally;11cases appear coagulant fuction obstacle;2cases are also diagnosed as preeclampsia, accounting for8.6%;2cases which appear DIC accounting for17.3%;21cases appear amniotic fluid abnormally, accounting for74%; There are22patients terminating pregnancy by cesarean section,and the other1patients did that by perineal operation.Conclusion:The characteristics of Gestational acute fatty liver are sudden onset, fast progress and diversiform clinieal manifestation. The early diagnosis, timely termination of Pregnancy and comprehensive coordination treatments of relevant clinical departments remain top priority in prevention and treatment department.In recent years,the treatment of AFLP had made great progress and the fetal death rate have dropped.The progress is due to early diagnosis and early treatments.
