The Effect Of Nicorandil On Ventricular Repolarization Indexes In Patients With Coronary Slow Flow

Objective:To observe the effects on ventricular repolarization indexes of slow coronary flow(CSF)by using the interval between the peak and end of T pave(Tp-e),QT interval(QT),corrected QT interval(QTc)and the ratio Tp-e inteval to QT interval(Tp-e/QT),the ratio of Tp-e interval to QTc interval(Tp-e/QTc).and to evaluate the effect of nicorandil on ventricular repolarization indexes(Tp-e,QT,QTc,Tp-e/QT,Tp-e/QTc)in patient with CSF.Method:97 patients underwent coronary angiography due to angina were enrolled from July 2016 to June 2017 in The Affiliated Hospital of Qiingdao University.Of those52 with coronary slow flow as CSF group,47 patients with normal coronary angiography as nomoal group;the patients of CSF group were randomly divided into routine treatment group and nicorandil group;all CSF patients were treated with conventional therapy,such as aspirin,statins,beta blockers or calcium antagonists,the patients in nicorandil group give nicorandil 5mg Tid treatment on the basis of routine treatment.The12-leads electrocardiography were used to evaluated patients before and 3 months after treatment,then measure an calculde the indexes of electrocardiogram.Result:Compared with the nomoal group,the general information,such as sex,hypertension,diabetes,dyslipidemia,of CSF group was no statistically significant difference(P<0.05);Tp-e,QT,QTc,Tp-e/QT and Tp-e/QTc as the indexes of ventricular repolarization were significantly prolonged in the slow flow group(115.5±15.6vs99.0±18.2,P < 0.001;428.2±31.8vs409.7±24.7,P=0.002;438.3±29.6vs423.6±27.3,P=0.013;0.271±0.039vs0.241±0.042,P < 0.001;0.265±0.038vs0.234±0.042,P < 0.001),the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in nicorandil group and routine treatment group in the general information and ECG indexes.After treatment,the ventricular repolarization indexes of the nicorandil group was significantly shorter(96.4±20.1vs119.4±19.4;398.4±35.0vs431.8±33.1;410.6±32.3vs438.9±29.6;0.243±0.053vs0.279±0.050;0.235±0.051vs0.274±0.048,P < 0.05),and tend to be normalcompared with the normol groupe;and the differences was statistically significant compared with the routine treatment group(P<0.05).Conclusion:CSF may be associated with prolonged ventricular repolarization indexes(Tp-e,QT,QTc,Tp-e/QT and Tp-e/QTc),nicorandil may be useful in restoration of these findings.
