Myocardial Function Changes In Patients With Coronary Slow Flow Left Anterior Descending Branch And The Therapeutic Effect Of Nicorandil

Objective: To evaluate the left ventricular global and regional function in patients with left anterior descending coronary slow flow and the therapeutic effects of Nicorandil.Methods: A coronary angiography examination was performed between January 2017 and September 2017 due to chest pain or chest distress,A total of 72 patients underwent coronary angiography with angina or chest tightnesswere enrolled.Of those 36 with left anterior descending coronary slow flow as CSF group,36 patients with normal coronary angiography as control group.Left ventricular end-diastolic volume(EDV),1eft ventricular end-systolic,volume,(ESV),left ventricular ejection fraction(LVEF),stroke volume(SV),E peak,A peak and E/A ratio were assessed using routine heartDoppler ultrasonic.Tomtec software was used to measure the global longitudinal strain values of left ventricle(GLS)andthe longitudinal strain(LS)values of 6 ventricular wall segments in the left ventricle,which were only supplied by LAD.The CSF group was taken orally 3 times a day,5mg per time,and after 3 months of regular drug use,the symptoms of chest pain or chest tightness were observed,and the above indicators were measured again.Results: There was no significant difference in EDV,ESV,SV and LVEF between the CSF group and the control group.Compared with the control group,the CSF group’s E peak、E/A ratio decreased and A peak increased.Ls in four of the six segments that single supplied by LAD decreased.After three months of treatment,77.8 percent of patients in the CSF group experienced complete remission of chest pain or chest tightness and significantly increased E peak and E/A ratio.The LS value increased in 4 segments,and the difference was significant.GLS value increased and the difference was significant.Conclusion: The myocardial diastolic function and systolic function of the patients with the coronary artery were reduced,indicating that the myocardial ischemia changes in the patients with the coronary artery slow flow group.Three-dimensional speckle tracking technology is the conventional ultrasonic cardiogram can more early detection of myocardial ischemia,After the regular application of Nicorandil for three months,The myocardial blood supply of the slow flow group was significantly improved,Nicorandil maybe an effective drug for treatment of coronary slow flow.
