Objective:To explore the feasibility of ultrasound-guided anterior sciatic nerve, femoral nerve block anesthesia combined with general anesthesia in lower extremity surgery of lumbar spine disease. Methods:From December 2016 to June 2017 in our hospital line of 80 cases of patients with lower extremity surgery of lumbar disease, were randomly divided into research group and the control group,40 cases, control group patients with pure general anesthesia, the team at the same time of general anesthesia combined ultrasound guided anterior sciatic nerve and femoral nerve block. The postoperative resuscitation time, the agitation situation and the dosage of anesthetic drugs were analyzed in the two groups.Note medicine before and after drug injection for 15 minutes,30 minutes,60 minutes, when the operation completed respectively on two groups of patients with heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP),blood oxygen saturation(SpO2)monitoring,to assess its hemodynamics. At the same time, the incidence of nausea and vomiting headache, chills, bradycardia, hypotension and other complications in the two groups were statistically analyzed. Results:In the study group, the dosage of anesthetic drugs was less than that of the control group, and the time and wake time of the patients was shorter than that in the control group, and the difference was statistically significant(P<0.05).The incidence of restlessness and restlessness in the study group were lower than that in the control group, and the differencewas statistically significant(P<0.05).Study groups of patients with drug injection after 15 minutes,30 minutes,60 minutes and after surgery of HR,SBP,DBP were significantly below the medicine before(P < 0.05),the control group were not significant with high level. Two group of patient with drug injection after 15 minutes,30 minutes,60 minutes and after surgery of SpO2 were significantly higher than before the drug injection(P < 0.05).The incidence of complications in the study group was 10.0%(4/40)significantly lower than that in the control group 42.5%(17/40)(P<0.05).Conclusion:Ultrasound guided by anterior sciatic nerve and femoral nerve block combined general anesthesia in patients with lumbar disease, can reduce the dosage of analgesic drugs, maintain the stability of intraoperative blood flow, improve the effect of analgesia, promote early extubation and activity,is an ideal way of lower limb fracture surgery anesthesia, and hemodynamics in patients with lower extremity surgery lumbar disease and complications has smaller influence.
