ObjectiveThe purpose of this study was to investigate the relationship between family management,parents coping strategy and asthma control in children with asthma,so as to provide more targeted health education and better family coping for nursing staff;and to help family maintain balance between daily life and disease management and promote asthma control.MethodsTotally 142 children with asthma and their parents were investigated in the outpatient clinic of a tertiary hospital of Qingdao during August 2016 to April 2017.The research tools include: the Family Management Measure(FaMM),the Coping Health Inventory for Parents(CHIP),Childhood Asthma Control Test(C-ACT),and detecting fractional nitric oxide concentration in exhaled breath(FENO)in children who met the exclusion criteria.Statistical software SPSS 17.0 was employed for data analysis.Descriptive analysis,one-way ANOVA,chi-square test,rank sum test,Pearson correlation analysis,multiple stepwise regression analysis were included.Results1.There were significant differences in FENO and FENO distribution with different asthma control groups(P?0.05).The value of FENO was higher and the distribution of high level FENO was more in the uncontrolled group of asthma.2.There were significant differences in FaMM with different asthma control(P?0.05).Child daily life,condition management ability and parent maturity were positively correlated with C-ACT and negatively correlated with FENO respectively(P ?0.01);Condition management effort,condition management difficulty and view of condition impact were negatively correlated with C-ACT and positively correlated with respectively(P?0.01).Multiple stepwise regression analysis showed that Child daily life,Condition management effort and view of condition impact had influence on C-ACTscore;while Condition management effort and view of condition impact had influence on FENO value.3.There were significant differences in CHIP with different asthma control(P ?0.05).The frequency and effect of each dimension of CHIP were positively correlated with C-ACT and negatively correlated with FENO respectively(P ? 0.01).Multiple stepwise regression analysis showed that the frequency and effect in the dimension of“maintaining family integration,cooperation and optimistic outlook”,the frequency in the dimension of “counseling with medical stuff and communicating with other parents”had influence on C-ACT score;while the effect in the dimension of “maintaining family integration,cooperation and optimistic outlook”had influence on FENO value.ConclusionFENO is correlated with the level of asthma control,and there is a certain degree of consistency in assessing asthma control.C-ACT combined with FENO can be more objective and comprehensive to assess asthma control in children.Each aspect of family management have an effect on asthma control in children.The higher condition management ability,better parent maturity,and the more recognition of the children's daily life benefit asthma control.The harder condition management difficulty and excessive view of condition impact and condition management effort disadvantages asthma control in children.Nursing staff should know all aspects of family management and put forward specific suggestions to promote asthma control.Parents coping strategy play a positive role in the control of pediatric asthma.It is helpful for parents to take active coping strategy to reduce the influence of disease to their family,to cope better,and then to promote the recovery of disease in their children..
