OBJECTIVE:To investigate the clinical efficacy and adverse reactions of tandospirone in the treatment of functional dyspepsia(FD)patients with anxiety,and to understand the changes of serum BDNF concentrations before and after anti-anxiety treatment in FD patients,and to explore the possible mechanism.METHODS:1.One hundred FD patients with anxiety were randomly divided into experimental group(53 cases)and placebo group(47 cases).In addition,serum of 100 healthy and age-matched volunteers was collected as control for FD patients.The experimental group was given tandospirone and the placebo group was given placebo treatment.2.The scores of gastrointestinal symptoms and anxiety symptoms before and after treatment in both groups were evaluated using the Gastrointestinal Symptom Score(GIS)and the Hamilton Anxiety Scale(HAMA).3.Serum of healthy volunteers and all patients(before and after treatment)were collected from and enzyme-linked immunosorbent assay was used to detect brain-derived neurotrophic factor(BDNF)concentration.RESULTS:1.After 8 weeks of treatment,GIS scores of both groups were lower than that before treatment(P<0.05),while the experimental group showed much more significant changes than that of the placebo group;According to the HAMA scores changes,the effective rate of anti-anxiety of the experimental group was significantly higher than placebo group(P<0.05).2.After 8 weeks of treatment,serum BDNF concentrations were observed significantly increased in experimental group than before treatment(P<0.05),instead of that in placebo group.3.Tandospirone was well tolerated,which showed similar low rate of adverse reactions with placebo treatment.CONCLUSION:1.Tandospirone treatment not only improved gastrointestinal symptoms of FD patients with anxiety,but also ameliorated their anxiety status.2.Serum BDNF concentration in FD patients was lower than healthy volunteers,and tandospirone increased serum BDNF levels of FD patients,which was negatively associated with GIS changes,suggesting that BDNF may become a serum marker for assessing the progression of FD patients.
