Objective1. To study the function of psychological factor of FD patients by evaluating their anxious and depressive scores2 To study the clinical significance of BDNF and CGRP of the subgroups of FD patients by exploring the change of these factors in their peripheral blood serum and gastric mucosa3 To investigate the relationship between BDNF、CGRP and psychosocial factors such as anxiety and depression.Methods1. From April 2014 to August 2014,50 FD patients who visited the department of gastroenterology of our hospital and met the Rome Ⅲ diagnosis criteria wereselected.According to their clinical features,they were divided into three subtypes,including 23 patients of PDS,11 patients of EPS,and 16 patients ofthe PDS overlapping EPS.At the same time,30 patients without FD and abnormalities which were detected in our physical examination centre wereselected as control group.2. Assessment was performed with Hamilton Anxiety Scale(HAMA)/Hamilton Depression Scale(HAMD) in all subjects,and compared the anxious and depressive position between FD patients group and the healthy control group and found out the difference among all the subtypes of FD patients.3. Collected the serum sample of the FD patients group and the healthy control group. Tested the BDNF and CGRP concentration of the samples using ELISA method.Compared the concentration between FD patient group and the healthy control group and found out the difference.Compared the concentration among all the subtypes of FD patients and analyzed the correlation between BDNF、CGRP and the subtypes’ anxious and depressive position.4. Immunohistochemical methods were used to detected the expression of BDNF,CGRP in gastric mucosa.The immunostaining densities of BDNF、CGRP in gastric mucosa were quantitatively assessed with the Image-pro plus6.0.The mean optical density(OD) of the positive areas was measured.The results were expressed as the exact value of the relative optical density units. Compared the results between FD patient group and the healthy control group and found out the difference.Compared the results among all the subtypes of FD patients and analyzed the correlation among BDNF、CGRP and the subtypes’ anxious and depressive position.5. Analyzed the correlation between the expression of BDNF in gastric mucosa and its expression in serum.6. The SPSSsoftware(version 16.0) was used in this study and all data given were expressed as mean values±standard deviation.The independent two-sample t-test was used for the continuous variables and the Chi-squared test was used for the categorical variables.For the multiple group parametric comparisons, we used one-way ANOVA to compare the mean of each group after the test of homogeneity of variane.Correlation between two parameters was assessed by Pearson correlation.Differences were considered significant when p<0.05.Results1. The average HAMA scores of the FD patienis was13.54±11.11 and the average HAMD seores was10.3±9.80.The avcrage seores of HAMA and HAMD were significantly higher in the group of EPS and PDS group than in the PDS group and the EPS group. The avcrage HAMD seores were significantly higher in the group of EPS than that in the PDS group.2. The average scores of HAMA and HAMD in the group of young and midlife people were significantly higher than that in the group of agedness(P<0.05)3. The serum BDNF concentration of FD group,PDS group,EPS group,EPS overlapping PDS group and the healthy control group was 8.25±6.49 ng/L, 8.17±6.29 ng/L,11.10±8.61 ng/L,5.77±4.32 ng/L,4.38±1.91 ng/L.The serum level of BDNF was significantly higher in FD patients than it in the normal controls(P<0.05).The serum level of BDNF was significantly higher in the PDS group、EPS group than that in the normal controls(P<0.05). However there were no significant difference between the EPS overlapping PDS group and the healthy control group.4. The serum CGRP concentration of FD group,PDS group,EPS group,EPS overlapping PDS group and the healthy control group was36.91±13.21ng/L, 30.34±9.08ng/L,35.75±10.53ng/L,46.54±14.60ng/L,20.06±7.92ng/L.The serum level of CGRP was significantly higher in FD patients and all the subtypes than it in the normal controls(P<0.05),The surem level of CGRP was significantly higher in EPS overlapping PDS group than it in PDS group and EPS group(P<0.05).5. The BDNF level of serum was significantly lower in FD patients with depression and anxiety than it in FD patients without depression and anxiety(P<0.05).The CGRP level of serum was significantly higher in FD patients with depression and anxiety than it in FD patients without depression and anxiety.6. The mean optical density of BDNF in gastric mucosa of FD group,the PDS group,EPS group,EPS overlapping PDS group and the healthy control group was 0.61±0.22,0.49±0.16,0.59±0.15,0.79±0.22,0.35±0.10 respectively.BDNF expressed in gastric mucosa was significantly higher in FD patients and all the subtypes than it in the normal controls(P<0.05).CGRP expressed in gastric mucosa was significantly higher in EPS overlapping PDS group than it in PDS group and EPS group.(P<0.05).7. The mean optical density of CGRP in gastric mucosa of FD group,PDS group,EPS group,EPS overlapping PDS group and the healthy control group was0.59±0.21,0.46±0.13,0.58±0.14,0.77±0.20,0.34±0.11.CGRP expressed in gastric mucosa was significantly higher in FD patients and all the subtypes than it in the normal controls(P<0.05).CGRP expressed in gastric mucosa was significantly higher in EPS overlapping PDS group than it in PDS group and EPS group.(P<0.05).8. The expression of BDNF in serum of FD patients was not correlated with its expression in gastric mucosa(r=0.021,p>0.05),but the expression of CGRP in serum of FD patients was positively correlated with its expression in gastric mucosa(r=0.901, p<0.05).Conclusion1. The study shows that all the subytpes of FD patients usually have anxiety and depression condition,especially the EPS overlapping PDS group,which indicates anxiety and depression are highly related to FD.2. The FD patients in young and midlife were easier to anxiety and depression state than older.3. The serum level of BDNF was significantly lower in FD patients with anxiety and depression,which prompted that the BDNF was concemed with the depressive and anxiety state in FD patients.The serum level of CGRP was significantly higher in FD patients with anxiety and depression,which prompted that the CGRP was concemed with the depressive and anxiety state in FD patients.4. The level of BDNF and CGRP in gastric mucosa was significantly higher in FD patients with anxiety and depression,which indicated that the BDNF and CGRP in gastric mucosa was concemed with the depressive and anxiety state in FD patients.5. The serum level of BDNF and CGRP was significantly higher in FD patients,which indicated that up-regulation BDNF,CGRP in serum play a important role in the occurrence of FD.6. The level of BDNF and CGRP in gastric mucosa was significantly higher in FD patients, which indicated that up-regulation of BDNF and CGRP expression in gastric mucosa plays an essential role in in the occurrence of FD.7. The expression of CGRP in serum can reflect its expression in gastric mucosa in FD.but the expression of BDNF in serum can not reflect its expression in gastric mucosa in FD.8. The BDNF,CGRP expression in gastric mucosa is significantly higher in the PDS overlapping EPS group than in the PDS group and the PDS group,which belongs to the more severe hypersensitivity,low grade inflammation,ect in the PDS overlapping EPS group.
