| Objective: To explore the effect of peripheral nerve block anesthesia on postoperative rehabilitation of patients with total knee arthroplasty by comparing peripheral nerve block anesthesia with combined lumbar and hard anesthesia.Method: 57 cases of the first total knee arthroplasty in August-2016 September 2015 were included in the study,of which 28 cases were operated by peripheral nerve block anesthesia(group A),and 29 cases were treated with combined lumbar and hard anesthesia(group B).The two groups of patients,sex,age,BMI,American anesthesiologist ASA classification,and other general data,the difference was not statistically significant(P>0.05),with comparability.The two groups of two groups of postoperative flow rate,total inpatient analgesic drug use(including opioids and non steroids),discharged knee joint HSS score,6h/12h/24 h resting VAS score,and 2/4/6 day joint passive activity were compared and analyzed.Result: There was no significant difference in the HSS score between the two groups and the postoperative drainage volume between the two groups(P > 0.05).The total morphine use equivalent A group(21.07 + 14.17)mg was less than that of group B(39.14 + 18.08)Mg,and the difference was statistically significant(P < 0.05);the use of fluprofen ester(7.75 + 2.07)x 50 mg was less than that of the B group(15.17 + 6.04)x 50 mg,and the difference was statistically significant(P < 0.05),and the postoperatively 6h/12h/24 h resting VAS score was(1.86 + 0.71),3.32 + 39.14 The difference was significantly lower than that in group B(3.48 + 1.1),(4.66 + 1.01),(4.52 + 0.87),and the difference was statistically significant(P < 0.05).The A group was(43.04 + 10.21),(83.21 +)and(98.75 + 9.30)degrees(43.04 + 10.21)and(98.75 + 9.30)degrees in group 2/4/6 on day 2/4/6.There was no significant difference in the sixth day(P < 0.05).There were 3 patients with nausea and vomiting in group A,11 in group B,the difference was statistically significant(P < 0.05),1 in group A had urinary retention,and 3 in group B had no statistical significance(P > 0.05).Conclusion: For patients with total knee arthroplasty,peripheral nerve block anesthesia compared with traditional lumbar epidural anesthesia can reduce the early postoperative pain and the amount of total analgesic drugs in the perioperative period,increase the degree of postoperative joint activity and reduce the incidence of postoperative nausea and vomiting. |
